# Miconazole Vaginal Pessary
## Overview
Miconazole is an imidazole antifungal agent. Vaginal pessaries are solid dosage forms inserted into the vagina for local treatment.
## Primary Indications
Treatment of vulvovaginal candidiasis (yeast infection).
## Adult Dosing
A single miconazole vaginal pessary (typically 100 mg, 200 mg, or 1200 mg depending on product formulation) is inserted vaginally once daily at bedtime. Treatment duration varies by strength:
* **100 mg pessary:** Insert one pessary vaginally once daily for 7 days.
* **200 mg pessary:** Insert one pessary vaginally once daily for 3 days.
* **1200 mg pessary:** Insert one pessary vaginally as a single dose.
Follow specific product labeling.
## Pediatric Dosing
Dosing in pediatric patients (under 12 years of age) is not well established and should be guided by prescriber judgment and product availability. Use with caution.
## Dose Adjustments
No dose adjustments are typically necessary based on renal or hepatic impairment due to minimal systemic absorption.
## Contraindications
* Hypersensitivity to miconazole or any component of the pessary formulation.
* Concurrent use with certain medications (see Drug Interactions).
## Adverse Effects
* **Common:** Vaginal burning, itching, irritation, redness. Pelvic cramps may occur.
* **Less Common:** Rash, hives.
## Key Drug Interactions
Miconazole is a moderate inhibitor of CYP3A4 and CYP2C9.
* **Warfarin:** Increased risk of bleeding. Monitor INR closely. Concurrent use is generally not recommended with the vaginal formulation.
* **Other CYP3A4 substrates:** Potentially increased concentrations of drugs like certain statins, calcium channel blockers, and immunosuppressants. While systemic absorption from vaginal pessaries is low, caution is advised.
## Monitoring
* Symptomatic improvement.
* Monitor for signs of hypersensitivity or severe local irritation.
## Clinical Pearls
* The pessary should be inserted as high as possible into the vagina, preferably at bedtime to minimize leakage and maximize contact time.
* Avoid intercourse and use of tampons or douches during treatment.
* The oil-based vehicle of the pessary may weaken latex condoms and diaphragms. Advise patients to use alternative birth control methods during and for at least 72 hours after treatment.
* If symptoms do not improve after the course of treatment, re-evaluate diagnosis and consider alternative therapy.
