# Miconazole Vaginal Pessary
## Overview
Miconazole is an imidazole antifungal agent.
## Primary Indications
Treatment of vulvovaginal candidiasis (yeast infection).
## Adult Dosing
A single vaginal insert (pessary) containing 100 mg miconazole nitrate is available and can be used as a one-time treatment. Alternatively, a 200 mg vaginal suppository can be used once, or a 100 mg vaginal suppository can be used for 7 days. Specific dosing and duration depend on the product formulation available and may be guided by local protocols.
## Pediatric Dosing
The safety and efficacy of miconazole vaginal pessaries in pediatric patients have not been established.
## Dose Adjustments
No dose adjustments are typically required for renal or hepatic impairment.
## Contraindications
Hypersensitivity to miconazole or any component of the formulation.
## Adverse Effects
Common adverse effects include vaginal burning, itching, irritation, and lower abdominal cramping.
## Key Drug Interactions
* **Warfarin:** Miconazole (especially oral formulations, but systemic absorption from vaginal use is possible) can inhibit CYP2C9, increasing warfarin levels and the risk of bleeding. Monitor INR closely if used concomitantly.
* **CYP3A4 Substrates:** Miconazole can inhibit CYP3A4, potentially increasing the concentration of drugs metabolized by this enzyme.
## Monitoring
* Symptomatic improvement.
* Monitor for signs of hypersensitivity or severe local irritation.
## Clinical Pearls
* Avoid intercourse during treatment.
* Miconazole vaginal products can weaken latex condoms and diaphragms, reducing their effectiveness. Consider alternative birth control methods during and for at least 72 hours after treatment.
* If symptoms persist after completing treatment, re-evaluate the diagnosis and consider alternative antifungal therapy.
