# Miconazole Vaginal Pessary
## Overview
Miconazole is an imidazole antifungal agent used to treat vulvovaginal candidiasis (yeast infections). It is available as a vaginal pessary (suppository) for localized treatment.
## Primary Indications
Treatment of symptomatic vulvovaginal candidiasis.
## Adult Dosing
* **100 mg vaginal pessary:** Insert 1 pessary vaginally at bedtime for 7 consecutive days.
* **200 mg vaginal pessary:** Insert 1 pessary vaginally at bedtime for 3 consecutive days.
* **High-strength (e.g., 1200 mg) vaginal suppository:** Insert 1 suppository vaginally at bedtime as a single dose.
Specific dosing regimens may vary based on product formulation and local protocol.
## Pediatric Dosing
Dosing for pediatric patients is not well-established. Use in pediatric patients should be based on clinical judgment and may require consultation with a pediatric specialist.
## Dose Adjustments
No dose adjustments are typically required for renal or hepatic impairment with vaginal administration due to minimal systemic absorption.
## Contraindications
Hypersensitivity to miconazole or any component of the formulation.
## Adverse Effects
Common adverse effects include vaginal itching, burning, or irritation. Allergic reactions are rare.
## Key Drug Interactions
* **Warfarin:** Miconazole can inhibit CYP3A4 and CYP2C9, potentially increasing warfarin levels and the risk of bleeding. Monitor INR closely if co-administered.
## Monitoring
* Assess for symptom relief (e.g., reduction in itching, burning, and discharge).
* Monitor for any signs of local irritation or allergic reaction.
* Monitor INR if co-administered with warfarin.
## Clinical Pearls
* Complete the full course of therapy as directed, even if symptoms improve earlier.
* Avoid intercourse during treatment.
* The pessary may weaken latex condoms and diaphragms; consider alternative birth control methods during treatment.
* If symptoms persist or worsen after completing therapy, re-evaluate for alternative diagnoses or resistant organisms.
