## Miconazole Vaginal Pessary
### Overview
Miconazole is an imidazole antifungal agent. A vaginal pessary is a solid dosage form inserted into the vagina that dissolves or disperses to release medication.
### Primary Indications
Treatment of vulvovaginal candidiasis (yeast infection).
### Adult Dosing
* **Dosage:** One 100 mg miconazole vaginal pessary inserted vaginally once daily for 7 days.
* **Maximum:** Not applicable for this specific formulation; follow package instructions.
### Pediatric Dosing
* Safety and efficacy in pediatric patients have not been established for the miconazole vaginal pessary.
### Dose Adjustments
* No specific dose adjustments are typically required based on renal or hepatic impairment for the vaginal formulation.
### Contraindications
* Hypersensitivity to miconazole or any component of the pessary.
### Adverse Effects
* **Common:** Vaginal burning, itching, irritation, pelvic pain, cramping.
* **Less Common:** Vulvar irritation.
### Key Drug Interactions
* **Warfarin:** Miconazole (especially oral or systemic) can inhibit CYP2C9, potentially increasing warfarin levels and INR. While systemic absorption from vaginal miconazole is minimal, caution is advised, and INR monitoring may be considered, particularly with prolonged use or in patients with risk factors.
### Monitoring
* Clinical improvement of symptoms (itching, burning, discharge).
* Monitor for signs of local irritation or allergic reaction.
### Clinical Pearls
* Complete the full course of treatment even if symptoms improve sooner.
* Avoid sexual intercourse during treatment.
* Pessaries may weaken latex condoms and diaphragms.
* If symptoms persist after treatment, re-evaluate the diagnosis and consider alternative therapy.
