# Miconazole Vaginal Pessary
## Overview
Miconazole is an imidazole antifungal agent. The vaginal pessary formulation is used for the treatment of vulvovaginal candidiasis (yeast infections).
## Primary Indications
Treatment of vaginal yeast infections caused by *Candida* species.
## Adult Dosing
* **Vulvovaginal Candidiasis:**
* One 200 mg vaginal pessary inserted vaginally at bedtime for 3 consecutive nights.
* Alternatively, one 100 mg vaginal pessary inserted vaginally at bedtime for 7 consecutive nights.
* Note: Combination products may also be available containing miconazole cream for external use.
## Pediatric Dosing
* Data on the use of miconazole vaginal pessaries in pediatric patients is limited. Prescribing should be based on clinical judgment and available data, often mirroring adult dosing for appropriate age groups, but caution is advised. Consult specific pediatric guidelines if available.
## Dose Adjustments
* No specific dose adjustments are typically required for renal or hepatic impairment for the vaginal pessary formulation, as systemic absorption is minimal.
## Contraindications
* Hypersensitivity to miconazole or any component of the formulation.
## Adverse Effects
* **Common:** Vaginal burning, itching, irritation. Pelvic cramps may occur.
* **Less Common:** Vulvar irritation, headache.
## Key Drug Interactions
* **Warfarin:** Miconazole (especially oral formulations, but caution with vaginal administration) can inhibit CYP3A4, potentially increasing warfarin levels and INR. Monitor INR closely if co-administered.
* **CYP3A4 Substrates:** Miconazole can inhibit CYP3A4, potentially increasing concentrations of other drugs metabolized by this enzyme. This is less likely with vaginal administration due to minimal systemic absorption, but caution is advised.
## Monitoring
* Symptom improvement (resolution of itching, burning, and discharge).
* If symptoms persist after treatment, reassessment for alternative diagnoses or resistant organisms is recommended.
## Clinical Pearls
* Ensure the pessary is fully inserted to maximize efficacy.
* Treatment should be completed even if symptoms improve before the end of the course.
* Avoid intercourse during treatment.
* The base of the pessary may cause some discomfort or leakage. Wearing a panty liner can help.
