# Miconazole Vaginal Pessary
## Overview
Miconazole is an imidazole antifungal agent. The vaginal pessary formulation is used for the topical treatment of vulvovaginal candidiasis (yeast infections).
## Primary Indications
Treatment of vulvovaginal candidiasis.
## Adult Dosing
* **100 mg vaginal pessary:** Insert one pessary intravaginally at bedtime for 7 consecutive days.
* **200 mg vaginal pessary:** Insert one pessary intravaginally at bedtime for 3 consecutive days.
* **200 mg vaginal suppository (may be used as a pessary):** Insert one suppository intravaginally at bedtime for 3 consecutive days.
* **For recurrent infections or severe cases, a longer duration of treatment may be necessary, as guided by clinical judgment.**
## Pediatric Dosing
* **Children 12 years and older:** Use adult dosing.
* **Children younger than 12 years:** Safety and efficacy have not been established. Consult with a pediatrician for appropriate dosing.
## Dose Adjustments
No specific dose adjustments are typically required for hepatic or renal impairment for vaginal formulations due to minimal systemic absorption.
## Contraindications
* Hypersensitivity to miconazole or any component of the formulation.
* Caution: Concurrent use with warfarin may increase INR; monitor INR closely.
## Adverse Effects
* **Common:** Vulvovaginal burning, itching, irritation, or discomfort. Abdominal cramping or pain.
* **Less Common:** Headache, rash.
* **Rare:** Allergic reactions.
## Key Drug Interactions
* **Warfarin:** Miconazole can inhibit CYP2C9, leading to increased warfarin levels and increased risk of bleeding. Avoid concurrent use or monitor INR very closely.
* **Other CYP3A4 substrates:** Miconazole can inhibit CYP3A4, potentially increasing the concentration of other drugs metabolized by this enzyme (e.g., certain statins, benzodiazepines, calcium channel blockers). Systemic miconazole has more significant interactions; however, caution is advised with prolonged vaginal use or in patients with significant systemic absorption potential.
## Monitoring
* Monitor for signs and symptoms of vulvovaginal candidiasis.
* Monitor for and report any signs of allergic reaction or significant local irritation.
* If concurrent warfarin therapy, monitor INR.
## Clinical Pearls
* Ensure complete insertion of the pessary for optimal efficacy.
* Advise patients to complete the full course of therapy, even if symptoms improve earlier.
* Avoid intercourse during treatment.
* Partner may require treatment if symptomatic.
* Pregnancy: While systemic absorption is minimal, consult with a healthcare provider before use during pregnancy, especially during the first trimester.
