# Miconazole Vaginal Pessary
## Overview
Miconazole is an imidazole antifungal agent. Its use in a vaginal pessary form is typically for the treatment of vaginal yeast infections (vulvovaginal candidiasis).
## Primary Indications
* Treatment of vulvovaginal candidiasis.
## Adult Dosing
The miconazole vaginal pessary is typically available in strengths of 100 mg, 200 mg, and 1200 mg.
* **100 mg pessary:** Insert one pessary vaginally at bedtime for 7 days.
* **200 mg pessary:** Insert one pessary vaginally at bedtime for 3 days.
* **1200 mg pessary:** Insert one pessary vaginally at bedtime as a single dose.
Specific duration and strength may vary based on product formulation and clinical guidelines.
## Pediatric Dosing
There is no established specific dosing for pediatric patients. Use in pediatric patients should be based on clinical judgment and relevant guidelines, with caution.
## Dose Adjustments
No dose adjustments are typically needed for renal or hepatic impairment due to minimal systemic absorption.
## Contraindications
* Hypersensitivity to miconazole or any component of the formulation.
## Adverse Effects
* **Common:** Vaginal burning, itching, irritation, lower abdominal cramps.
* **Less Common:** Headache, rash.
## Key Drug Interactions
* **Warfarin:** Miconazole (especially oral but potentially topical) can inhibit CYP2C9, increasing warfarin levels and risk of bleeding. Monitor INR closely if coadministered.
* **CYP3A4 Substrates:** Miconazole can inhibit CYP3A4. Although systemic absorption is low with vaginal administration, potential for interaction exists, especially with prolonged or widespread use.
## Monitoring
* Monitor for symptom resolution (itching, burning, discharge).
* Monitor for adverse effects.
* Monitor INR if used concurrently with warfarin.
## Clinical Pearls
* Vaginal pessaries are inserted directly into the vagina.
* Treatment duration depends on the strength of the pessary used.
* Avoid sexual intercourse during treatment.
* Consider recommending cotton underwear and avoiding douches or harsh soaps to prevent recurrence.
* If symptoms persist after treatment, further evaluation for other causes or resistant organisms may be necessary.
