# Miconazole Vaginal Pessary
## Overview
Miconazole is an imidazole antifungal agent. The vaginal pessary formulation is used for the treatment of vulvovaginal candidiasis (yeast infections).
## Primary Indications
Vulvovaginal candidiasis (yeast infections) caused by susceptible Candida species.
## Adult Dosing
* **Single Dose Treatment:** One 1200 mg miconazole nitrate vaginal suppository (pessary) inserted high into the vagina at bedtime.
* **Three-Day Treatment:** One 200 mg miconazole nitrate vaginal suppository inserted high into the vagina at bedtime for 3 consecutive nights.
* **Seven-Day Treatment:** One 100 mg miconazole nitrate vaginal suppository inserted high into the vagina at bedtime for 7 consecutive nights.
*Note: The specific strength and duration of treatment may vary based on product availability and clinical judgment.*
## Pediatric Dosing
Dosing for pediatric patients has not been established for the vaginal pessary formulation. Use in pediatric patients is generally not recommended unless specifically directed by a healthcare provider.
## Dose Adjustments
No specific dose adjustments are typically required for renal or hepatic impairment for vaginal administration.
## Contraindications
* Hypersensitivity to miconazole or any component of the formulation.
## Adverse Effects
Common side effects include vaginal burning, itching, irritation, and pelvic cramps.
## Key Drug Interactions
* **CYP3A4 Inhibitors:** Miconazole is a potent inhibitor of CYP3A4. Systemic absorption from vaginal administration is generally low, but caution is advised if used concurrently with medications metabolized by CYP3A4, especially those with a narrow therapeutic index (e.g., certain statins, anticoagulants, immunosuppressants). Avoid concomitant use with oral miconazole or other strong CYP3A4 inhibitors.
* **Warfarin:** Potential for increased INR and bleeding risk. Monitor INR closely if used concurrently.
## Monitoring
* Clinical signs and symptoms of vulvovaginal candidiasis (e.g., itching, burning, discharge).
* For patients on warfarin, monitor INR.
## Clinical Pearls
* Ensure the pessary is fully inserted into the vagina.
* Treatment may cause some leakage; wearing a sanitary pad can help.
* Complete the full course of treatment, even if symptoms improve earlier.
* Avoid intercourse during treatment to prevent reinfection or transmission.
* If symptoms do not improve after completing treatment, or if they worsen, consult a healthcare provider.
* The safety and efficacy of miconazole vaginal pessaries in pregnant women have not been fully established. Consult a healthcare provider for guidance during pregnancy.
