## Miconazole Vaginal Pessary
### Overview
Miconazole is an imidazole antifungal agent used to treat vaginal candidiasis (yeast infections). It is available in various vaginal formulations, including pessaries.
### Primary Indications
* Treatment of vulvovaginal candidiasis.
### Adult Dosing
* **100 mg vaginal pessary:** Insert 1 pessary vaginally at bedtime for 7 consecutive days.
* **200 mg vaginal pessary:** Insert 1 pessary vaginally at bedtime for 3 consecutive days.
* **Combination packs (e.g., 1200 mg single-dose pessary):** Insert 1 pessary vaginally as a single dose.

### Pediatric Dosing
* **Children 12 years and older:** Use adult dosing.
* **Children younger than 12 years:** Safety and efficacy have not been established. Consult with a pediatrician for appropriate management.
### Dose Adjustments
No dose adjustments are typically required for renal or hepatic impairment when used as a vaginal pessary due to minimal systemic absorption.
### Contraindications
* Hypersensitivity to miconazole or any component of the formulation.
### Adverse Effects
* **Common:** Vaginal burning, itching, irritation, and vulvar edema.
* **Less Common:** Pelvic cramps, headache.
### Key Drug Interactions
* **Warfarin:** Miconazole (particularly oral formulations, but caution with vaginal products) can inhibit CYP2C9, potentially increasing warfarin levels and INR. Monitor INR closely if co-administered.
* **CYP3A4 substrates:** Miconazole can inhibit CYP3A4. While systemic absorption is minimal with vaginal use, caution is advised with drugs that have a narrow therapeutic index and are primarily metabolized by CYP3A4.
### Monitoring
* Clinical signs and symptoms of vaginal candidiasis (e.g., pruritus, erythema, discharge).
* Monitor for signs of local irritation or hypersensitivity.
### Clinical Pearls
* Ensure complete insertion of the pessary for optimal drug delivery.
* Avoid sexual intercourse during treatment to prevent potential irritation and to ensure efficacy.
* Inform patients that treatment may need to be repeated if symptoms persist or recur.
* For recurrent or complicated infections, further diagnostic evaluation may be warranted.
