# Miconazole Vaginal Pessary
## Overview
Miconazole is an imidazole antifungal agent. The vaginal pessary formulation is used for the treatment of vulvovaginal candidiasis (yeast infections).
## Primary Indications
Treatment of vulvovaginal candidiasis.
## Adult Dosing
* **Option 1 (Single Dose):** One 1200 mg vaginal suppository inserted intravaginally.
* **Option 2 (3-Day Regimen):** One 200 mg vaginal suppository inserted intravaginally once daily for 3 days.
* **Option 3 (7-Day Regimen):** One 100 mg vaginal suppository inserted intravaginally once daily for 7 days.
The choice of regimen may depend on patient preference and severity of symptoms.
## Pediatric Dosing
* **Children ≥ 12 years:** Use adult dosing.
* **Children < 12 years:** Safety and efficacy have not been established. Consult local protocol or pediatrician.
## Dose Adjustments
No dose adjustments are typically required for renal or hepatic impairment with vaginal administration.
## Contraindications
* Hypersensitivity to miconazole or any component of the formulation.
* Concomitant use with certain CYP3A4 substrates (e.g., simvastatin, lovastatin) due to potential for increased miconazole levels and risk of myopathy.
## Adverse Effects
* **Common:** Vaginal burning, itching, irritation, pelvic pain, cramping.
* **Less Common:** Headache, rash.
## Key Drug Interactions
* **CYP3A4 Inhibitors:** Miconazole is a moderate inhibitor of CYP3A4. Coadministration with drugs metabolized by CYP3A4 (e.g., warfarin, certain statins, calcium channel blockers) may increase plasma concentrations of these drugs and potentiate their toxicity. Concurrent use of miconazole vaginal products with oral miconazole or other strong CYP3A4 inhibitors is generally not recommended.
## Monitoring
* Symptom resolution.
* Signs of local irritation or allergic reaction.
## Clinical Pearls
* Vaginal pessaries should be inserted deep into the vagina, preferably at bedtime.
* Complete the full course of treatment, even if symptoms improve earlier.
* Avoid intercourse during treatment to prevent potential reinfection and transmission.
* Miconazole can weaken latex condoms and diaphragms, potentially leading to breakage.
