## Miconazole Vaginal Pessary
### Overview
Miconazole is an imidazole antifungal agent. Vaginal pessaries are solid dosage forms inserted into the vagina.
### Primary Indications
Treatment of vulvovaginal candidiasis (yeast infection).
### Adult Dosing
* **Formulations:** Available as 100 mg, 200 mg, or 1200 mg vaginal suppositories (pessaries).
* **100 mg:** Insert one 100 mg pessary vaginally at bedtime for 7 days.
* **200 mg:** Insert one 200 mg pessary vaginally at bedtime for 3 days.
* **1200 mg:** Insert one 1200 mg pessary vaginally as a single dose.
### Pediatric Dosing
* Data for pediatric use of miconazole vaginal pessaries is limited. Safety and efficacy have not been established in children. Use should be based on clinical judgment.
### Dose Adjustments
* No specific dose adjustments are required for renal or hepatic impairment.
### Contraindications
* Hypersensitivity to miconazole or any component of the formulation.
### Adverse Effects
* **Common:** Vulvovaginal burning, itching, irritation, redness, or pain. Headache.
* **Less Common:** Abdominal pain, rash.
### Key Drug Interactions
* **Warfarin:** Miconazole (especially oral or prolonged vaginal use) can inhibit CYP2C9, potentially increasing warfarin levels and INR. Monitor INR closely if coadministered.
* **CYP3A4 Substrates:** Miconazole can inhibit CYP3A4, potentially increasing the levels of drugs metabolized by this enzyme (e.g., certain statins, calcium channel blockers, immunosuppressants). Caution and monitoring may be warranted.
### Monitoring
* Monitor for signs and symptoms of vaginal infection.
* Monitor for adverse effects.
* If coadministered with warfarin, monitor INR regularly.
### Clinical Pearls
* Vaginal pessaries should be inserted at bedtime to minimize leakage.
* Treatment duration is typically 1, 3, or 7 days depending on the product strength.
* Avoid intercourse during treatment.
* Some formulations may weaken latex condoms and diaphragms; consider alternative methods of contraception if necessary.
