## Miconazole Vaginal Pessary
### Overview
Miconazole is an imidazole antifungal agent. This information pertains to its use in a vaginal pessary formulation, typically for the treatment of vaginal yeast infections (vulvovaginal candidiasis).
### Primary Indications
* Treatment of vulvovaginal candidiasis (yeast infections).
### Adult Dosing
* **100 mg vaginal pessary:** Insert 1 pessary vaginally once daily at bedtime for 7 days.
* **200 mg vaginal pessary:** Insert 1 pessary vaginally once daily at bedtime for 3 days.
* **1200 mg vaginal pessary:** Insert 1 pessary vaginally as a single dose at bedtime.
### Pediatric Dosing
* Safety and efficacy in pediatric patients have not been established for vaginal pessary formulations.
### Dose Adjustments
* No dose adjustments are typically required for renal or hepatic impairment when used as a vaginal pessary due to minimal systemic absorption.
### Contraindications
* Hypersensitivity to miconazole or any component of the formulation.
### Adverse Effects
* **Common:** Vaginal burning, itching, irritation, or discomfort. Lower abdominal cramping.
* **Less Common:** Headache, rash.
### Key Drug Interactions
* **Warfarin:** Miconazole (especially oral formulations, but topical/vaginal may have theoretical risk) can inhibit CYP2C9, potentially increasing warfarin's anticoagulant effect. Monitor INR closely if used concurrently with warfarin.
* **CYP3A4 Substrates:** Miconazole can inhibit CYP3A4. While systemic absorption is minimal with vaginal pessaries, caution should be exercised with co-administration of drugs that are narrow therapeutic index CYP3A4 substrates, though clinical significance is unlikely.
### Monitoring
* Monitor for symptom resolution.
* If symptoms do not improve after completing treatment, or if they worsen, re-evaluate for alternative diagnoses or resistant organisms.
### Clinical Pearls
* The pessary should be inserted as deeply as possible into the vagina, preferably at bedtime to minimize leakage and maximize contact time.
* Complete the full course of treatment even if symptoms improve earlier.
* Avoid sexual intercourse during treatment.
* Partner treatment is generally not recommended unless the partner also has symptoms of a yeast infection.
* Concurrent use of condoms or diaphragms may be affected due to the potential for miconazole to weaken latex.
