# Miconazole Vaginal Pessary
## Overview
Miconazole is an imidazole antifungal agent. The vaginal pessary formulation is used for the topical treatment of vulvovaginal candidiasis (yeast infections).
## Primary Indications
* Treatment of vulvovaginal candidiasis.
## Adult Dosing
* **Intravaginal:** One miconazole 200 mg vaginal suppository inserted into the vagina once daily at bedtime for 3 consecutive nights.
* Alternatively, one miconazole 100 mg vaginal suppository inserted into the vagina once daily at bedtime for 7 consecutive nights may be used.
* Another option is one miconazole 1200 mg vaginal suppository inserted into the vagina once at bedtime.
## Pediatric Dosing
Data for pediatric use of miconazole vaginal pessaries is limited. Consult product labeling or local institutional guidelines.
## Dose Adjustments
No dose adjustments are typically necessary for renal or hepatic impairment with topical vaginal administration.
## Contraindications
* Hypersensitivity to miconazole or any component of the formulation.
* Use with oral or topical midazolam due to potential for prolonged sedation and respiratory depression.
* Use with cisapride.
## Adverse Effects
* **Common:** Vaginal burning, itching, irritation.
* **Less Common:** Pelvic cramps.
## Key Drug Interactions
* **CYP3A4 Inhibitors:** Miconazole is a potent inhibitor of CYP3A4. Co-administration with drugs metabolized by CYP3A4 (e.g., warfarin, certain statins, immunosuppressants) may increase plasma concentrations and risk of toxicity. Close monitoring and dose adjustments of the interacting drug may be necessary.
* **Midazolam and Triazolam:** Concurrent use can lead to prolonged and exaggerated sedation and respiratory depression. Avoid concurrent use.
* **Cisapride:** Concurrent use is contraindicated due to increased risk of serious cardiac events.
## Monitoring
* Monitor for signs and symptoms of vaginal irritation or allergic reaction.
* Assess for symptom improvement.
## Clinical Pearls
* Ensure complete insertion of the suppository.
* It is recommended to use the product at bedtime to minimize leakage and maximize contact time.
* Although systemic absorption is minimal, women taking warfarin should be advised to report any signs of bleeding or bruising.
* Complete the full course of treatment, even if symptoms improve earlier.
