# Miconazole Vaginal Pessary
## Overview
Miconazole is an imidazole antifungal agent. Vaginal pessaries are solid dosage forms inserted into the vagina for local treatment.
## Primary Indications
Treatment of vulvovaginal candidiasis (yeast infection).
## Adult Dosing
Typically, a single 1200 mg miconazole vaginal suppository (pessary) is inserted high into the vagina at bedtime.
Alternatively, a 200 mg miconazole vaginal suppository can be used nightly for 3 consecutive nights.
## Pediatric Dosing
Data on the use of miconazole vaginal pessaries in pediatric patients is limited. Use in patients under 12 years of age should be at the discretion of a healthcare provider. If used, dosing typically follows adult recommendations.
## Dose Adjustments
No dose adjustments are typically required for renal or hepatic impairment when used as a vaginal pessary due to minimal systemic absorption.
## Contraindications
Hypersensitivity to miconazole or any component of the formulation.
## Adverse Effects
Common: Vaginal burning, itching, irritation, and cramping.
Less Common: Rash, hives.
## Key Drug Interactions
Miconazole is a potent inhibitor of CYP3A4. While systemic absorption from vaginal administration is minimal, caution should be exercised with coadministration of drugs metabolized by CYP3A4, especially those with a narrow therapeutic index (e.g., warfarin, certain statins, calcium channel blockers). However, significant drug interactions are unlikely with typical vaginal use.
## Monitoring
Monitor for symptom relief. If symptoms persist after completing treatment, re-evaluate for alternative diagnoses or resistant organisms.
## Clinical Pearls
Ensure the pessary is inserted as high as possible for optimal efficacy.
Avoid sexual intercourse during treatment.
May weaken latex condoms and diaphragms.
Treatment duration is typically short (1-3 days), leading to good compliance.
