## Miconazole Vaginal Pessary
### Overview
Miconazole is an imidazole antifungal agent. It is used in vaginal pessary form for the treatment of vaginal candidiasis (yeast infections).
### Primary Indications
* Treatment of vulvovaginal candidiasis (yeast infection).
### Adult Dosing
* **100 mg vaginal pessary:** Insert one pessary vaginally once daily at bedtime for 7 days.
* **200 mg vaginal pessary:** Insert one pessary vaginally once daily at bedtime for 3 days.
* **100 mg/5 g vaginal cream pessary:** Insert one 100 mg dose (one applicatorful) vaginally once daily at bedtime for 7 days.
* **200 mg/5 g vaginal cream pessary:** Insert one 200 mg dose (one applicatorful) vaginally once daily at bedtime for 3 days.
*Note: Specific treatment duration and regimen may vary based on product formulation and local protocol.*
### Pediatric Dosing
* Data for pediatric use of miconazole vaginal pessaries is limited. Consult specific product labeling or pediatric guidelines. Generally, topical antifungals are used cautiously in young children.
### Dose Adjustments
* No specific dose adjustments are typically required for hepatic or renal impairment with topical vaginal formulations.
### Contraindications
* Hypersensitivity to miconazole or any component of the formulation.
### Adverse Effects
* **Common:** Vaginal burning, itching, irritation, abdominal cramping, pelvic pain, headache.
* **Less Common:** Rash, urticaria.
### Key Drug Interactions
* **CYP3A4 Inhibitor:** Miconazole is a potent inhibitor of CYP3A4. Concurrent use with oral miconazole or high-dose vaginal miconazole may increase plasma concentrations of drugs metabolized by CYP3A4 (e.g., warfarin, certain statins, calcium channel blockers, immunosuppressants). While systemic absorption from vaginal formulations is generally low, caution is advised, especially with prolonged use or if other risk factors for drug interactions are present.
### Monitoring
* Symptom relief (e.g., reduction in itching, burning, discharge).
* Monitor for local irritation or signs of hypersensitivity.
### Clinical Pearls
* Ensure complete insertion of the pessary for optimal efficacy.
* Avoid sexual intercourse during treatment to prevent potential reinfection or irritation.
* Miconazole vaginal cream formulations may weaken latex condoms and diaphragms; concurrent use should be avoided.
* Treatment should continue for the full prescribed duration, even if symptoms improve sooner.
