# Miconazole Vaginal Pessary
## Overview
Miconazole is an imidazole antifungal agent. The vaginal pessary formulation is designed for localized treatment of vaginal fungal infections.
## Primary Indications
Treatment of vulvovaginal candidiasis (yeast infection).
## Adult Dosing
* **100 mg pessary:** Insert 1 pessary vaginally at bedtime for 7 consecutive nights.
* **200 mg pessary:** Insert 1 pessary vaginally at bedtime for 3 consecutive nights.
* **Single-dose 1200 mg pessary:** Insert 1 pessary vaginally as a single dose.
## Pediatric Dosing
Safety and efficacy in pediatric patients have not been established for the vaginal pessary formulation.
## Dose Adjustments
No dose adjustments are necessary based on renal or hepatic impairment.
## Contraindications
* Hypersensitivity to miconazole or any component of the formulation.
## Adverse Effects
Common adverse effects include vaginal itching, burning, and irritation. Pelvic pain and cramping may also occur.
## Key Drug Interactions
* **Warfarin:** Miconazole, particularly oral formulations, can significantly increase the anticoagulant effect of warfarin. While systemic absorption from vaginal pessaries is generally low, caution is advised, especially with prolonged use or in patients with risk factors for bleeding. Monitor INR closely.
* **CYP3A4 Substrates:** Miconazole is a moderate inhibitor of CYP3A4. Concomitant use with CYP3A4 substrates (e.g., certain statins, calcium channel blockers, benzodiazepines) may lead to increased plasma concentrations and potential toxicity. Systemic absorption from vaginal pessaries is typically minimal, limiting this risk.
## Monitoring
* Monitor for symptom improvement.
* Assess for signs of local irritation or hypersensitivity.
* If using concurrently with warfarin, monitor INR.
## Clinical Pearls
* Complete the full course of treatment as prescribed, even if symptoms improve earlier.
* Avoid intercourse during treatment to prevent potential reinfection of a partner and to avoid irritating the inflamed vaginal tissues.
* The pessary may weaken latex condoms and diaphragms.
