# Miconazole Vaginal Pessary
## Overview
Miconazole is an imidazole antifungal agent. The vaginal pessary formulation is used for the treatment of vulvovaginal candidiasis (yeast infections).
## Primary Indications
Treatment of vulvovaginal candidiasis (yeast infection).
## Adult Dosing
* **100 mg vaginal pessary:** Insert 1 pessary vaginally at bedtime for 7 consecutive days.
* **200 mg vaginal pessary:** Insert 1 pessary vaginally at bedtime for 3 consecutive days.
* **1200 mg vaginal pessary:** Insert 1 single-dose pessary vaginally at bedtime.
## Pediatric Dosing
Data for pediatric use of miconazole vaginal pessaries is limited. Caution is advised, and dosing should be guided by a healthcare provider.
## Dose Adjustments
No dose adjustments are typically required for renal or hepatic impairment for the vaginal pessary formulation.
## Contraindications
* Hypersensitivity to miconazole or any component of the formulation.
## Adverse Effects
* **Common:** Vulvovaginal burning or itching, pelvic cramps, lower abdominal pain.
* **Less Common:** Rash, hives, headache, nausea.
## Key Drug Interactions
* **Warfarin:** Miconazole vaginal products may interact with warfarin. Monitor INR closely if used concurrently.
* **CYP3A4 substrates:** Concurrent use with other CYP3A4 substrates (e.g., certain statins, calcium channel blockers, benzodiazepines) may increase the risk of toxicity due to inhibition of CYP3A4. While systemic absorption from vaginal miconazole is low, caution is still warranted.
## Monitoring
* Symptomatic improvement.
* Monitor for any signs of allergic reaction or severe local irritation.
## Clinical Pearls
* Advise patients to complete the full course of treatment even if symptoms improve sooner.
* Avoid intercourse during treatment to prevent potential reinfection and partner transmission (although transmission risk is likely low).
* Some formulations may be associated with oil-based residue; recommend the use of panty liners.
* The 1200 mg single-dose pessary offers convenience for patients who may have difficulty adhering to multi-day regimens.
