# Miconazole Vaginal Pessary
## Overview
Miconazole is an imidazole antifungal agent. Vaginal pessaries are solid dosage forms inserted into the vagina for local treatment.
## Primary Indications
Treatment of vaginal yeast infections (vulvovaginal candidiasis).
## Adult Dosing
The typical dosage is one miconazole vaginal pessary inserted into the vagina at bedtime. Treatment durations vary by product formulation:
* **100 mg pessary:** Insert one pessary nightly for 7 nights.
* **200 mg pessary:** Insert one pessary nightly for 3 nights.
* **100 mg vaginal suppository:** Insert one suppository nightly for 7 nights. (Note: Suppositories are similar to pessaries in function for this indication).
## Pediatric Dosing
The safety and efficacy of miconazole vaginal pessaries have not been established in pediatric patients. Use in this population is generally not recommended unless specifically directed by a healthcare provider.
## Dose Adjustments
No dose adjustments are typically required for renal or hepatic impairment when used as a vaginal pessary due to minimal systemic absorption.
## Contraindications
* Hypersensitivity to miconazole or any component of the formulation.
## Adverse Effects
Common adverse effects include vaginal burning, itching, irritation, and lower abdominal cramping.
## Key Drug Interactions
Systemic absorption is minimal; therefore, significant drug interactions are unlikely with vaginal administration. However, concurrent use of miconazole oral gel with certain medications metabolized by CYP3A4 (e.g., warfarin, some statins, benzodiazepines) can lead to increased plasma concentrations and toxicity. While vaginal absorption is less, caution is advised if a patient is using other miconazole products or is on systemic therapy with these interacting drugs.
## Monitoring
* Monitor for symptom improvement.
* Assess for local irritation or allergic reactions.
## Clinical Pearls
* Complete the full course of therapy even if symptoms improve earlier.
* Avoid intercourse during treatment to prevent potential irritation or transmission to a partner.
* If symptoms do not improve after completing the course, or if symptoms worsen, re-evaluate the diagnosis and consider alternative therapies.
* Some formulations may weaken latex condoms and diaphragms; concurrent use is not recommended.
