## Miconazole Vaginal Pessary
### Overview
Miconazole is an imidazole antifungal agent. The vaginal pessary formulation is used for the topical treatment of vaginal yeast infections (vulvovaginal candidiasis).
### Primary Indications
* Treatment of vulvovaginal candidiasis (yeast infection).
### Adult Dosing
* **Option 1:** One 100 mg vaginal pessary inserted vaginally at bedtime for 7 consecutive nights.
* **Option 2:** One 200 mg vaginal pessary inserted vaginally at bedtime for 3 consecutive nights.
* **Option 3:** One 1200 mg vaginal pessary inserted vaginally as a single dose.
The specific product and duration of therapy should be determined by the healthcare provider based on patient factors and symptom severity.
### Pediatric Dosing
Safety and efficacy in pediatric patients under 12 years of age have not been established for this formulation. Dosing for children 12 years and older is the same as adult dosing.
### Dose Adjustments
No dose adjustments are typically required based on renal or hepatic impairment due to minimal systemic absorption.
### Contraindications
* Hypersensitivity to miconazole or any component of the formulation.
### Adverse Effects
* **Common:** Vaginal burning, itching, irritation, or discomfort. Pelvic cramps.
* **Less Common:** Headache, rash.
### Key Drug Interactions
* **Warfarin:** Miconazole can inhibit CYP2C9, potentially increasing warfarin levels and the risk of bleeding. Concurrent use should be approached with caution, and INR monitoring is essential.

### Monitoring

### Clinical Pearls
* Complete the full course of treatment as prescribed, even if symptoms improve sooner.
* Avoid sexual intercourse during treatment to prevent potential reinfection or partner transmission, and to avoid potential irritation from the medication.
* The pessary may weaken latex condoms and diaphragms. Consider alternative birth control methods during treatment.
* The medication may cause some vaginal discharge or residue. Use of a panty liner is recommended.
