### Miconazole Vaginal
#### Overview
Miconazole is an imidazole antifungal agent.
#### Primary Indications
Treatment of vulvovaginal candidiasis (yeast infection).
#### Adult Dosing
Available in various strengths and dosage forms (cream, suppositories). Common regimens include:
* **1-day treatment:** One applicatorful of 2% cream or one 200 mg suppository at bedtime.
* **3-day treatment:** One applicatorful of 2% cream or one 100 mg suppository at bedtime.
* **7-day treatment:** One applicatorful of 2% cream or one 100 mg suppository at bedtime.
#### Pediatric Dosing
Dosing for pediatric patients is not well established. Use with caution and consider the adult regimens with appropriate medical supervision.
#### Dose Adjustments
No specific dose adjustments are necessary for renal or hepatic impairment due to minimal systemic absorption.
#### Contraindications
Hypersensitivity to miconazole or any component of the formulation.
#### Adverse Effects
Common: Vaginal burning, itching, irritation.
Less Common: Abdominal pain, headache, rash.
#### Key Drug Interactions
Miconazole is a moderate inhibitor of CYP3A4 and CYP2C9. Concomitant use with drugs metabolized by these enzymes (e.g., warfarin, certain statins, benzodiazepines) may increase the risk of toxicity. Topical vaginal administration is unlikely to cause significant systemic drug interactions, but caution is advised, especially with prolonged or extensive use.
#### Monitoring
* Monitor for signs and symptoms of vaginal candidiasis.
* Assess for local irritation or allergic reactions.
#### Clinical Pearls
* Complete the full course of treatment, even if symptoms improve.
* Avoid sexual intercourse during treatment to prevent reinfection and potential irritation.
* Miconazole can weaken latex condoms and diaphragms, making them less effective.
* If symptoms do not improve after completing treatment or if they worsen, re-evaluate for alternative diagnoses or resistant organisms.
