# Miconazole Vaginal Pessary
## Overview
Miconazole is an imidazole antifungal agent. The vaginal pessary formulation is used for the treatment of vaginal yeast infections (vulvovaginal candidiasis).
## Primary Indications
* Treatment of vulvovaginal candidiasis (yeast infections).
## Adult Dosing
A single vaginal pessary (typically 100 mg or 200 mg, depending on the product) is inserted intravaginally at bedtime. Follow product-specific instructions for the duration of treatment (usually one dose).
## Pediatric Dosing
Use in pediatric patients is generally not recommended due to lack of specific studies and potential for increased systemic absorption. Consult a pediatrician for appropriate management.
## Dose Adjustments
No dose adjustments are typically necessary for renal or hepatic impairment when used as a vaginal pessary due to minimal systemic absorption.
## Contraindications
* Hypersensitivity to miconazole or any component of the formulation.
## Adverse Effects
* **Common:** Vaginal burning, itching, irritation, vulvar discomfort.
* **Less Common:** Abdominal pain, headache.
## Key Drug Interactions
* **CYP3A4 Inhibitor:** Miconazole is a potent inhibitor of CYP3A4. Systemic absorption, although minimal with vaginal use, could theoretically increase the risk of toxicity with concomitant use of CYP3A4 substrates (e.g., warfarin, certain statins, calcium channel blockers). Close monitoring is advised if co-administration is unavoidable.
## Monitoring
* Monitor for signs and symptoms of vaginal candidiasis resolution.
* Assess for and manage any local irritation or hypersensitivity reactions.
## Clinical Pearls
* Ensure the pessary is inserted as high as possible into the vagina for optimal efficacy.
* Treatment is typically a single dose, but some products may be formulated for multiple days. Always follow the specific product labeling.
* Avoid intercourse during treatment to prevent potential transmission and to ensure product efficacy.
* If symptoms do not improve within 3 days or worsen, patients should seek medical attention.
