# Miconazole Vaginal Pessary
## Overview
Miconazole is an imidazole antifungal agent. The vaginal pessary formulation is intended for the treatment of vaginal yeast infections.
## Primary Indications
Treatment of vulvovaginal candidiasis (yeast infections).
## Adult Dosing
* **100 mg pessary:** Insert one pessary vaginally at bedtime for 7 days.
* **200 mg pessary:** Insert one pessary vaginally at bedtime for 3 days.
* **100 mg pessary:** Insert one pessary vaginally at bedtime for 3 days. (Some products may offer this option, consult product labeling).
## Pediatric Dosing
Safety and efficacy in pediatric patients (under 12 years) have not been established for the vaginal pessary formulation.
## Dose Adjustments
No specific dose adjustments are typically required for hepatic or renal impairment with vaginal administration, as systemic absorption is minimal.
## Contraindications
Hypersensitivity to miconazole or any component of the formulation.
## Adverse Effects
Common: Vaginal burning, itching, irritation, redness, pelvic cramps.
Less Common: Headache, rash.
## Key Drug Interactions
* **Warfarin:** Miconazole can inhibit CYP2C9, potentially increasing warfarin levels and the risk of bleeding. Monitor INR closely.
* **CYP3A4 substrates:** Miconazole can inhibit CYP3A4, potentially increasing the levels of other drugs metabolized by this enzyme (e.g., certain calcium channel blockers, benzodiazepines, statins). Use with caution and monitor for adverse effects.
## Monitoring
* Clinical improvement of symptoms (itching, burning, discharge).
* Monitor for signs of hypersensitivity or severe local irritation.
* For patients on warfarin, monitor INR.
## Clinical Pearls
* Complete the full course of treatment, even if symptoms improve earlier.
* Avoid intercourse during treatment.
* The oil-based pessary may weaken latex condoms and diaphragms. Consider alternative birth control methods during treatment and for 2-3 days after completion.
* If symptoms persist or worsen after completing the course, re-evaluate for alternative diagnoses or resistant organisms.
