# Miconazole Vaginal Pessary
## Overview
Miconazole is an imidazole antifungal agent used to treat vulvovaginal candidiasis. The vaginal pessary is a solid dosage form inserted into the vagina.
## Primary Indications
Treatment of vulvovaginal candidiasis (yeast infections).
## Adult Dosing
* **100 mg vaginal pessary:** Insert one pessary vaginally at bedtime for 7 consecutive days.
* **200 mg vaginal pessary:** Insert one pessary vaginally at bedtime for 3 consecutive days.
* **100 mg/6.5% vaginal suppository:** Insert one suppository vaginally at bedtime for 7 consecutive days. (Note: This is a different formulation, but often presented similarly for short-course therapy).
## Pediatric Dosing
Dosing for pediatric patients is not well-established for the vaginal pessary formulation. Use in pediatric patients should be guided by clinical judgment and the product's prescribing information. Topical antifungals are generally preferred for localized infections in children.
## Dose Adjustments
No specific dose adjustments are typically required for renal or hepatic impairment.
## Contraindications
* Hypersensitivity to miconazole or any component of the formulation.
* Concurrent use with certain CYP3A4 substrates (e.g., oral midazolam, triazolam, simvastatin, lovastatin) due to potential for increased plasma concentrations and serious adverse events.
## Adverse Effects
* **Common:** Vaginal burning, itching, irritation, vulvar edema, pelvic pain.
* **Less Common:** Headache, rash.
## Key Drug Interactions
* **CYP3A4 Inhibitors:** Miconazole is a potent inhibitor of CYP3A4. Concomitant use with drugs metabolized by CYP3A4 may lead to increased plasma concentrations and toxicity. This is primarily a concern with systemic absorption, but caution is advised. Avoid oral midazolam, triazolam, simvastatin, and lovastatin.
* **Warfarin:** Miconazole may increase the anticoagulant effect of warfarin. Monitor INR closely.
## Monitoring
* Monitor for signs and symptoms of vulvovaginal candidiasis to assess treatment efficacy.
* Monitor for adverse effects, particularly local irritation or signs of systemic absorption (rare).
* If concomitant warfarin therapy, monitor INR regularly.
## Clinical Pearls
* This formulation is for vaginal use only. Do not administer orally.
* Avoid intercourse and the use of tampons or douches during treatment.
* Treatment may be continued during menstruation.
* If symptoms do not improve after completing the course of therapy, reassess diagnosis and consider alternative treatment.
