## Miconazole Vaginal Pessary
### Overview
Miconazole is an imidazole antifungal agent. The vaginal pessary formulation is used for the topical treatment of vulvovaginal candidiasis.
### Primary Indications
Vulvovaginal candidiasis (yeast infection).
### Adult Dosing
* **Single dose:** Insert one 200 mg vaginal pessary intravaginally at bedtime.
* **Three-day regimen:** Insert one 100 mg vaginal pessary intravaginally at bedtime for 3 consecutive nights.
* **Seven-day regimen:** Insert one 100 mg vaginal pessary intravaginally at bedtime for 7 consecutive nights.
The choice of regimen may depend on product availability and prescriber preference.
### Pediatric Dosing
Safety and efficacy in pediatric patients have not been established for the vaginal pessary formulation.
### Dose Adjustments
No specific dose adjustments are necessary for hepatic or renal impairment.
### Contraindications
* Hypersensitivity to miconazole or any component of the formulation.
### Adverse Effects
* **Common:** Vulvovaginal burning, itching, irritation.
* **Less Common:** Pelvic pain, rash.
### Key Drug Interactions
* **CYP3A4 Inhibitors:** Miconazole is a moderate inhibitor of CYP3A4. Systemic absorption from vaginal administration is generally low, but caution is advised with co-administration of other drugs metabolized by CYP3A4, especially those with a narrow therapeutic index (e.g., warfarin, some immunosuppressants).
### Monitoring
* Monitor for symptom improvement.
* Assess for signs of local irritation or allergic reaction.
### Clinical Pearls
* The pessary should be inserted as high as possible into the vagina.
* Treatment may be continued during menstruation.
* Complete the full course of treatment even if symptoms improve sooner.
* Use tampons, douches, and other vaginal products with caution during treatment as they may affect the pessary's efficacy.
* Partners may require treatment if symptomatic.
