# Miconazole Vaginal Pessary
## Overview
Miconazole is an imidazole antifungal agent. Vaginal pessaries are inserted directly into the vagina for localized treatment.
## Primary Indications
Treatment of vulvovaginal candidiasis (yeast infection).
## Adult Dosing
* **100 mg vaginal pessary:** Insert 1 pessary vaginally once daily at bedtime for 7 days.
* **200 mg vaginal pessary:** Insert 1 pessary vaginally once daily at bedtime for 3 days.
* **One-day treatment (e.g., 1200 mg vaginal suppository):** Insert 1 suppository vaginally once at bedtime.
* **Note:** Specific duration and strength may vary based on product availability and local protocol.
## Pediatric Dosing
Safety and efficacy in pediatric patients below the age of 12 years have not been established. Use in this age group is not recommended.
## Dose Adjustments
No dose adjustments are typically required for hepatic or renal impairment for vaginal formulations due to minimal systemic absorption.
## Contraindications
Hypersensitivity to miconazole or any component of the formulation.
## Adverse Effects
Common: Vaginal burning, itching, irritation, and lower abdominal cramping.
Less Common: Headache, rash.
## Key Drug Interactions
* **CYP3A4 Inhibitors:** Miconazole is a potent inhibitor of CYP3A4. Concomitant use with substrates of CYP3A4 (e.g., certain statins, benzodiazepines, anticoagulants like warfarin) can increase the plasma concentrations of these drugs, potentially leading to toxicity. The risk is generally considered low with vaginal formulations due to limited systemic absorption, but caution is advised, especially with prolonged use or in patients with risk factors.
## Monitoring
* Monitor for signs and symptoms of vaginal infection.
* Assess for and report any adverse effects.
## Clinical Pearls
* Complete the full course of treatment, even if symptoms improve earlier.
* Avoid sexual intercourse during treatment to prevent reinfection and potential transmission.
* Miconazole vaginal products can weaken latex condoms and diaphragms, potentially leading to breakage. Use alternative birth control methods if necessary.
* Wash hands before and after insertion.
