# Miconazole Vaginal Pessary
## Overview
Miconazole is an imidazole antifungal agent. The vaginal pessary is a solid dosage form inserted into the vagina for localized treatment.
## Primary Indications
Vulvovaginal candidiasis (yeast infection).
## Adult Dosing
* **100 mg pessary:** Insert one pessary vaginally at bedtime for 7 consecutive days.
* **200 mg pessary:** Insert one pessary vaginally at bedtime for 3 consecutive days.
* **100 mg pessary (single dose):** Insert two pessaries vaginally at bedtime once. (Note: This presentation may be less common or subject to local availability/protocol).
## Pediatric Dosing
Dosing in pediatric patients is not well established. Use with caution and consider adult dosing as a reference, with appropriate consultation.
## Dose Adjustments
No dose adjustments are necessary for hepatic or renal impairment.
## Contraindications
Hypersensitivity to miconazole or any component of the formulation.
## Adverse Effects
Common adverse effects include vaginal burning, itching, irritation, and vulvar/vaginal pain. Less common effects may include abdominal cramping, nausea, and rash.
## Key Drug Interactions
* **CYP3A4 Inhibitors:** Miconazole is a moderate inhibitor of CYP3A4. Concomitant use with drugs metabolized by CYP3A4 (e.g., certain statins, benzodiazepines, anticoagulants like warfarin) may increase plasma concentrations of these drugs, potentially leading to toxicity. Advise patients to avoid or use caution with these concomitant medications.
* **Tacrolimus:** Miconazole can significantly increase tacrolimus levels. Concomitant use is generally not recommended.
## Monitoring
* Monitor for relief of symptoms (itching, burning, discharge).
* Assess for local adverse reactions.
## Clinical Pearls
* Complete the full course of therapy, even if symptoms improve sooner.
* Avoid sexual intercourse during treatment to prevent potential irritation and reduced efficacy.
* Pessaries may cause leakage; consider using a pad.
* Latex condoms and diaphragms may be damaged by the oil-based vehicle in some miconazole vaginal products, potentially leading to contraceptive failure or increased risk of STI transmission. Advise patients to use alternative birth control methods during treatment and for at least 72 hours after completion.
