# Miconazole Vaginal Pessary
## Overview
Miconazole is an imidazole antifungal agent. The vaginal pessary formulation is designed for local treatment of vaginal yeast infections.
## Primary Indications
Vulvovaginal candidiasis (yeast infection).
## Adult Dosing
* **100 mg pessary:** Insert one pessary vaginally at bedtime for 7 days.
* **200 mg pessary:** Insert one pessary vaginally at bedtime for 3 days.
* **Higher strength pessaries (e.g., 1200 mg):** Insert one pessary vaginally as a single dose.
## Pediatric Dosing
Safety and efficacy in pediatric patients have not been established for the vaginal pessary formulation. Use is generally not recommended in this population.
## Dose Adjustments
No dose adjustments are necessary for renal or hepatic impairment due to minimal systemic absorption.
## Contraindications
* Hypersensitivity to miconazole or any component of the formulation.
* Concurrent use with certain medications (see Key Drug Interactions).
## Adverse Effects
* **Common:** Vaginal burning, itching, irritation.
* **Less Common:** Pelvic cramps, rash.
## Key Drug Interactions
* **CYP3A4 Inhibitors:** Miconazole is a potent inhibitor of CYP3A4. Concurrent use with CYP3A4 substrates (e.g., certain statins, benzodiazepines, opioids, calcium channel blockers, anticoagulants like warfarin) can lead to increased plasma concentrations and potential toxicity. Avoid concurrent use or closely monitor for signs of toxicity if coadministration is unavoidable.
* **Warfarin:** Increased anticoagulant effect. Monitor INR closely.
## Monitoring
* Monitor for symptom relief (e.g., reduced itching, burning, discharge).
* Monitor for adverse effects.
## Clinical Pearls
* Complete the full course of therapy, even if symptoms improve sooner.
* Avoid sexual intercourse during treatment.
* If symptoms persist after treatment or recur, consult a healthcare provider.
* Pregnancy: While systemic absorption is low, it is generally prudent to use the shortest duration regimen and avoid the higher strength single-dose products if possible during the first trimester. Consult prescribing information and healthcare provider.
