# Miconazole Vaginal Pessary
## Overview
Miconazole is an imidazole antifungal agent effective against a variety of yeasts and fungi. The vaginal pessary formulation is used for the treatment of vulvovaginal candidiasis.
## Primary Indications
* Treatment of vulvovaginal candidiasis (yeast infection).
## Adult Dosing
* **100 mg vaginal pessary:** Insert 1 pessary vaginally at bedtime for 7 consecutive days.
* **200 mg vaginal pessary:** Insert 1 pessary vaginally at bedtime for 3 consecutive days.
* **100 mg vaginal suppository:** Insert 1 suppository vaginally at bedtime for 7 consecutive days.
* **Higher strength formulations (e.g., 1200 mg single-dose ovule) may also be available, follow product labeling.**
## Pediatric Dosing
* Data on the use of miconazole vaginal pessaries in pediatric patients is limited. Consult current pediatric guidelines or product information.
## Dose Adjustments
* No specific dose adjustments are typically required for renal or hepatic impairment.
## Contraindications
* Hypersensitivity to miconazole or any component of the formulation.
* Use with oral ketoconazole or other CYP3A4 inhibitors, as miconazole is a potent CYP3A4 inhibitor and can significantly increase plasma concentrations of other drugs metabolized by this enzyme.
## Adverse Effects
* **Common:** Vaginal irritation, itching, burning, redness, cramping, lower abdominal pain.
* **Less Common:** Headache, rash, urticaria.
## Key Drug Interactions
* **CYP3A4 Inhibitors:** Miconazole is a potent inhibitor of CYP3A4. Concomitant use with drugs metabolized by CYP3A4 (e.g., certain statins, benzodiazepines, anticoagulants like warfarin, calcium channel blockers, immunosuppressants like cyclosporine and tacrolimus) can lead to increased plasma concentrations of these drugs, potentially causing toxicity. While systemic absorption from vaginal administration is generally low, caution is still advised, particularly with prolonged or extensive use.
* **Oral Anticoagulants:** Increased risk of bleeding due to potentiation of warfarin's effect.
## Monitoring
* Monitor for signs and symptoms of vaginal infection resolution.
* Monitor for adverse effects, particularly local irritation.
* If using concurrently with other medications metabolized by CYP3A4, monitor for signs of toxicity of those medications.
## Clinical Pearls
* Treatment duration depends on the strength of the pessary/suppository chosen. Complete the full course of therapy even if symptoms improve earlier.
* Avoid intercourse during treatment.
* Lubricants may be used with miconazole products if needed, but avoid oil-based lubricants which may degrade latex condoms.
* Pessaries may leave a residue and can weaken latex condoms and diaphragms.
