# Miconazole Vaginal Pessary
## Overview
Miconazole is a synthetic imidazole antifungal agent. Vaginal pessaries are designed for local intravaginal administration.
## Primary Indications
Treatment of vulvovaginal candidiasis (yeast infections).
## Adult Dosing
* **100 mg pessary:** Insert 1 pessary intravaginally at bedtime for 7 consecutive days.
* **200 mg pessary:** Insert 1 pessary intravaginally at bedtime for 3 consecutive days.
* **100 mg vaginal cream (often supplied with applicators for intravaginal use):** Administer 1 applicatorful (approximately 5g) intravaginally at bedtime for 7 consecutive days.
## Pediatric Dosing
Dosing in pediatric patients is not well established. Consult product labeling or specific pediatric guidelines. Use with caution and consider efficacy and safety in this population.
## Dose Adjustments
No specific dose adjustments are typically required for hepatic or renal impairment due to minimal systemic absorption.
## Contraindications
Hypersensitivity to miconazole or any component of the formulation.
## Adverse Effects
* **Common:** Vaginal burning, itching, irritation, lower abdominal cramping.
* **Less Common:** Rash, headache.
## Key Drug Interactions
* **CYP3A4 Inhibitors:** Miconazole is a potent inhibitor of CYP3A4. Concomitant use with medications metabolized by CYP3A4 (e.g., certain statins, benzodiazepines, calcium channel blockers, anticoagulants like warfarin) can increase plasma concentrations of these drugs, potentially leading to toxicity. While systemic absorption of vaginal miconazole is low, caution is advised, particularly with prolonged use or in patients with risk factors for systemic drug exposure.
* **Warfarin:** Increased anticoagulant effect. Monitor INR closely.
## Monitoring
* Monitor for signs and symptoms of vulvovaginal candidiasis.
* Monitor for adverse effects.
* If using concurrently with warfarin, monitor INR.
## Clinical Pearls
* Treatment duration depends on the strength of the pessary/cream. Adherence to the full course is important for efficacy.
* Avoid sexual intercourse during treatment.
* The partner may require treatment if symptomatic (balanitis).
* Complete symptom resolution may take several days after the course is finished.
* If symptoms persist or worsen, re-evaluate for other causes or resistant organisms.
