# Miconazole Vaginal Pessary
## Overview
Miconazole is a synthetic imidazole antifungal agent. Vaginal pessaries are inserted into the vagina for localized treatment of vulvovaginal candidiasis.
## Primary Indications
Treatment of vulvovaginal candidiasis (yeast infections).
## Adult Dosing
* **100 mg vaginal pessary:** Insert one pessary vaginally at bedtime for 7 consecutive days.
* **200 mg vaginal pessary:** Insert one pessary vaginally at bedtime for 3 consecutive days.
* **Intravaginal gel (not a pessary, but a common alternative):** 5 grams (containing 100 mg miconazole nitrate) inserted vaginally at bedtime for 7 days.
## Pediatric Dosing
The safety and efficacy of miconazole vaginal pessaries in pediatric patients (under 12 years of age) have not been established. Use in this population is generally not recommended.
## Dose Adjustments
No specific dose adjustments are typically required for hepatic or renal impairment due to minimal systemic absorption.
## Contraindications
Hypersensitivity to miconazole or any component of the formulation.
## Adverse Effects
* **Common:** Vaginal burning, itching, irritation, redness, discharge, pelvic cramps.
* **Less Common:** Headache, rash.
## Key Drug Interactions
* **Warfarin:** Miconazole can inhibit CYP2C9 and CYP3A4, potentially increasing warfarin levels and the risk of bleeding. Monitor INR closely if coadministered.
* **CYP3A4 Substrates:** Miconazole may increase concentrations of other drugs metabolized by CYP3A4 (e.g., certain statins, calcium channel blockers, benzodiazepines). Use with caution and consider dose reduction of the interacting drug if necessary.
## Monitoring
* Monitor for relief of symptoms (itching, burning, discharge).
* Assess for and manage adverse effects.
* Monitor INR if coadministered with warfarin.
## Clinical Pearls
* Pessaries should be inserted as high into the vagina as possible.
* Treatment should be completed for the full duration prescribed, even if symptoms improve earlier.
* Avoid intercourse and do not use tampons or douches during treatment.
* If symptoms do not improve after completing treatment or if they worsen, re-evaluate for alternative diagnoses or resistant organisms.
* There is some uncertainty regarding the efficacy of single-dose 200 mg pessaries compared to the 7-day regimen for some patients.
