# Miconazole Vaginal Pessary
## Overview
Miconazole is an imidazole antifungal agent. The vaginal pessary formulation is used for the topical treatment of vulvovaginal candidiasis.
## Primary Indications
Treatment of vulvovaginal candidiasis (yeast infection).
## Adult Dosing
* **100 mg pessary:** Insert 1 pessary vaginally at bedtime for 7 days.
* **200 mg pessary:** Insert 1 pessary vaginally at bedtime for 3 days.
* **100 mg pessary:** Insert 2 pessaries vaginally at bedtime for 3 days. (This option is less common but may be available).
## Pediatric Dosing
Dosing in pediatric patients (under 12 years) is not established for the vaginal pessary formulation. Consult a pediatrician.
## Dose Adjustments
No specific dose adjustments are typically required for renal or hepatic impairment with vaginal administration due to minimal systemic absorption.
## Contraindications
* Hypersensitivity to miconazole or any component of the formulation.
## Adverse Effects
* **Common:** Vulvar/vaginal irritation, burning sensation, itching.
* **Less Common:** Pelvic cramps.
## Key Drug Interactions
* **Warfarin:** Miconazole (especially oral or systemic forms) can inhibit CYP2C9, leading to increased warfarin levels and bleeding risk. While vaginal absorption is minimal, caution is advised, especially with prolonged use or in patients with impaired vaginal mucosa. Monitor INR closely.
## Monitoring
* Symptomatic improvement.
* If symptoms persist or worsen after completing the course, re-evaluate diagnosis and consider alternative treatment.
## Clinical Pearls
* Complete the full course of treatment, even if symptoms improve earlier.
* Avoid intercourse during treatment.
* The pessary may weaken latex condoms and diaphragms.
