# Miconazole Vaginal Pessary
## Overview
Miconazole vaginal pessaries are a topical antifungal medication used to treat vaginal yeast infections (vulvovaginal candidiasis).
## Primary Indications
* Vulvovaginal candidiasis caused by *Candida* species.
## Adult Dosing
* **100 mg pessary:** Insert 1 pessary vaginally at bedtime for 7 days.
* **200 mg pessary:** Insert 1 pessary vaginally at bedtime for 3 days.
* **100 mg pessary (less common formulation):** Insert 1 pessary vaginally at bedtime for 3 days. (Verify specific formulation instructions).
## Pediatric Dosing
* No specific pediatric dosing is established. Treatment for children is generally not recommended unless under the direct supervision of a healthcare provider.
## Dose Adjustments
* No dose adjustments are typically required based on renal or hepatic impairment for vaginal formulations.
## Contraindications
* Hypersensitivity to miconazole or any component of the formulation.
## Adverse Effects
* **Common:** Vulvovaginal burning or itching, abdominal cramps, pelvic pain, headache.
* **Less Common:** Rash, urticaria.
## Key Drug Interactions
* **Warfarin:** Miconazole, especially oral formulations, can significantly increase warfarin levels. While the systemic absorption of vaginal miconazole is low, caution and monitoring are advised, particularly with prolonged use or in patients with compromised mucosal integrity.
* **CYP3A4 Inhibitors:** Miconazole is a potent inhibitor of CYP3A4. Although systemic absorption from vaginal use is minimal, potential for interaction with CYP3A4 substrates (e.g., certain statins, benzodiazepines, calcium channel blockers) exists, especially with prolonged or extensive use.
## Monitoring
* Clinical improvement of symptoms (itching, burning, discharge).
* If symptoms persist after treatment, consider recurrent infections or alternative diagnoses.
## Clinical Pearls
* Complete the full course of therapy, even if symptoms improve earlier.
* Avoid intercourse during treatment to prevent irritation and potential transmission.
* The pessary may cause some discharge or leakage; using a panty liner is recommended.
* Some formulations may weaken latex condoms and diaphragms. Advise patients to use alternative contraception during treatment.
* Effectiveness against non-*albicans* *Candida* species may vary.
