## Pediatric Dosing
Safety and efficacy in pediatric patients have not been established for the vaginal pessary formulation.
## Dose Adjustments
No specific dose adjustments are typically required for hepatic or renal impairment due to minimal systemic absorption.
## Contraindications
Hypersensitivity to miconazole or any component of the formulation.
## Adverse Effects
Common: Vaginal burning, itching, irritation.
Less Common: Abdominal pain, headache, rash.
## Key Drug Interactions
* **CYP3A4 Inhibitors:** Miconazole is a potent CYP3A4 inhibitor. Concomitant use with substrates of CYP3A4 (e.g., certain statins, warfarin, benzodiazepines, immunosuppressants) can increase systemic concentrations and risk of toxicity. Avoid concurrent use where possible, or monitor closely.
* **Warfarin:** Miconazole may increase the anticoagulant effect of warfarin. Monitor INR closely.
## Monitoring
* Clinical signs and symptoms of vulvovaginal candidiasis.
* For patients on interacting medications (e.g., warfarin), monitor relevant laboratory parameters (e.g., INR).
## Clinical Pearls
* Complete the full course of therapy as directed, even if symptoms improve.
* Avoid intercourse during treatment.
* Miconazole can weaken latex condoms and diaphragms, potentially leading to breakage or leakage. Use alternative contraception during treatment and for at least 72 hours after completion.
* If symptoms do not improve after completing therapy or if they worsen, consult a healthcare provider for re-evaluation.
