# Miconazole Vaginal Suppository
## Overview
Miconazole is a synthetic imidazole antifungal agent. It works by inhibiting the synthesis of ergosterol, a vital component of fungal cell membranes.
## Primary Indications
* Treatment of vulvovaginal candidiasis (yeast infection).
## Adult Dosing
* **100 mg vaginal suppository:** Insert one suppository vaginally at bedtime for 7 consecutive days.
* **200 mg vaginal suppository:** Insert one suppository vaginally at bedtime for 3 consecutive days.
* **10% vaginal cream:** (If applicable, though the request specifies pessary/suppository) Apply the contents of one applicator (approximately 5 grams) vaginally at bedtime for 7 consecutive days.
## Pediatric Dosing
* **Children 12 years and older:** Use adult dosing.
* **Children younger than 12 years:** Safety and efficacy have not been established. Consult with a pediatrician for appropriate management.
## Dose Adjustments
* **Renal impairment:** No dose adjustment is typically required.
* **Hepatic impairment:** No specific dose adjustment is typically required for vaginal administration, but caution may be advised in severe hepatic dysfunction.
## Contraindications
* Hypersensitivity to miconazole or any component of the formulation.
## Adverse Effects
* **Common:** Vaginal burning, itching, irritation, pelvic cramps.
* **Less Common:** Headache, rash.
## Key Drug Interactions
* **CYP3A4 Inhibitors:** Miconazole is a potent inhibitor of CYP3A4. Concurrent use with drugs metabolized by CYP3A4 (e.g., warfarin, certain statins, calcium channel blockers, cyclosporine) may increase plasma concentrations of these drugs, potentially leading to toxicity. Close monitoring and dose adjustments of the co-administered drug may be necessary. *Note: Systemic absorption from vaginal administration is generally low, but the potential for interaction still exists, especially with prolonged use or in patients with compromised mucosal integrity.*
## Monitoring
* Monitor for symptom resolution (e.g., reduction in itching, burning, and discharge).
* Assess for signs of local irritation or allergic reaction.
## Clinical Pearls
* Use the applicator provided for accurate insertion.
* It is recommended to administer the suppository at bedtime to minimize leakage and maximize contact time.
* Complete the full course of treatment, even if symptoms improve sooner.
* Avoid intercourse during treatment as the medication may weaken latex condoms and diaphragms.
* If symptoms do not improve after completing treatment, or if they worsen, re-evaluate the diagnosis and consider alternative therapies.
