# Miconazole Vaginal Pessary
## Overview
Miconazole is an imidazole antifungal agent that inhibits the synthesis of ergosterol, a critical component of fungal cell membranes. This leads to increased cell membrane permeability and fungal cell death.
## Primary Indications
Treatment of vulvovaginal candidiasis (yeast infection).
## Adult Dosing
* **100 mg vaginal pessary:** Insert one pessary vaginally at bedtime for 7 consecutive days.
* **200 mg vaginal pessary:** Insert one pessary vaginally at bedtime for 3 consecutive days.
* **100 mg/5g vaginal cream:** (While not a pessary, often used interchangeably) Insert one applicatorful (approximately 5g) vaginally at bedtime for 7 days.
## Pediatric Dosing
* **Children 12 years and older:** Use adult dosing. Safety and efficacy in children under 12 years have not been established for vaginal pessaries.
## Dose Adjustments
No specific dose adjustments are typically required for hepatic or renal impairment for vaginal formulations.
## Contraindications
Known hypersensitivity to miconazole or any component of the formulation.
## Adverse Effects
Common: Vaginal burning, itching, irritation, abdominal cramps, pelvic pain, headache.
Less Common: Rash, hives.
## Key Drug Interactions
* **CYP3A4 Inhibitors:** Miconazole is a potent inhibitor of CYP3A4. Concomitant use with CYP3A4 substrates (e.g., simvastatin, atorvastatin, certain opioids) can increase plasma concentrations of these drugs, potentially leading to toxicity. Concurrent use of oral miconazole with certain CYP3A4 substrates is contraindicated. While systemic absorption of vaginal miconazole is minimal, caution is still advised, especially with prolonged or widespread use.
* **Warfarin:** Miconazole may potentiate the effect of warfarin. Monitor INR closely if co-administered.
## Monitoring
* Monitor for signs and symptoms of vaginal candidiasis.
* Assess for and report any adverse effects.
## Clinical Pearls
* Complete the full course of treatment, even if symptoms improve sooner.
* Avoid sexual intercourse during treatment to prevent reinfection and potential irritation.
* If symptoms do not improve after completing the course, or if they worsen, consult a healthcare provider to confirm the diagnosis and rule out other causes.
* Pregnancy: While generally considered low risk for topical vaginal use, consult a healthcare provider for specific recommendations during pregnancy.
