# Miconazole vaginal pessary
## Overview
Miconazole is an imidazole antifungal agent used for the topical treatment of vulvovaginal candidiasis.
## Primary Indications
Treatment of vulvovaginal candidiasis (yeast infection) caused by *Candida* species.
## Adult Dosing
* **100 mg vaginal pessary:** Insert 1 pessary vaginally at bedtime for 7 consecutive days.
* **200 mg vaginal pessary:** Insert 1 pessary vaginally at bedtime for 3 consecutive days.
* **150 mg vaginal pessary:** Insert 1 pessary vaginally at bedtime for 2 consecutive days.
The choice of regimen (3-day or 7-day) may depend on patient preference and severity of symptoms.
## Pediatric Dosing
Dosing for children is not well established for the vaginal pessary formulation. Consult pediatric guidelines or a specialist.
## Dose Adjustments
No dose adjustments are typically required for renal or hepatic impairment due to minimal systemic absorption.
## Contraindications
Hypersensitivity to miconazole or any component of the formulation.
## Adverse Effects
* **Common:** Vaginal burning, itching, irritation, cramping.
* **Less Common:** Rash, hives.
## Key Drug Interactions
* **CYP3A4 Inhibitors:** Miconazole is a moderate inhibitor of CYP3A4. Concurrent use with drugs metabolized by CYP3A4 (e.g., warfarin, certain statins, some calcium channel blockers) may increase plasma concentrations of these drugs, potentially leading to toxicity. Use with caution or avoid.
* **Warfarin:** Increased INR has been reported. Monitor INR closely if used concurrently.
## Monitoring
* Symptom resolution (itching, burning, discharge).
* Monitor for signs of hypersensitivity or local irritation.
## Clinical Pearls
* Treatment duration should be completed even if symptoms improve early.
* Avoid intercourse during treatment to prevent potential transmission and reinfection.
* Some miconazole vaginal products may weaken latex condoms, reducing their effectiveness. Advise patients to use alternative birth control methods during treatment and for at least 72 hours after completion.
* If symptoms persist or worsen after treatment, re-evaluate for alternative diagnoses or resistant organisms.
