# Miconazole Vaginal Pessary
## Overview
Miconazole is a synthetic imidazole antifungal agent. Vaginal pessaries are designed for local treatment of vaginal yeast infections.
## Primary Indications
Vulvovaginal candidiasis (yeast infection).
## Adult Dosing
* **100 mg pessary:** Insert one pessary vaginally at bedtime for 7 consecutive nights.
* **200 mg pessary:** Insert one pessary vaginally at bedtime for 3 consecutive nights.
* **100 mg pessary:** Insert one pessary vaginally at bedtime for a single dose. (Less commonly used, may be less effective for some presentations).
## Pediatric Dosing
Dosing has not been established for pediatric patients. Use in pediatric patients is generally not recommended unless directed by a healthcare provider, and specific dosing would be extrapolated from adult guidelines or individual clinical judgment.
## Dose Adjustments
No dose adjustments are typically necessary based on renal or hepatic impairment for vaginal administration, as systemic absorption is minimal.
## Contraindications
* Known hypersensitivity to miconazole or any component of the formulation.
## Adverse Effects
Common adverse effects include vaginal burning, itching, and irritation. Less common effects may include lower abdominal cramping and headache.
## Key Drug Interactions
* **Warfarin:** Miconazole can inhibit the metabolism of warfarin, potentially increasing the International Normalized Ratio (INR) and risk of bleeding. Close monitoring of INR is recommended if concomitant use is necessary.
* **CYP3A4 Substrates:** Miconazole is a potent inhibitor of CYP3A4. Concurrent administration with other drugs metabolized by CYP3A4 may increase the plasma concentrations of these drugs. While systemic absorption is low with vaginal administration, caution is advised.
## Monitoring
* Monitor for improvement in symptoms of vulvovaginal candidiasis.
* Assess for signs of local irritation or allergic reaction.
* If using concurrently with warfarin, monitor INR.
## Clinical Pearls
* Pessaries should be inserted as high as possible into the vagina.
* Treatment should be completed even if symptoms improve before the prescribed duration.
* Avoid sexual intercourse during treatment.
* Consider advising patients to use sanitary pads to prevent leakage of the medication.
* If symptoms do not improve after completing treatment, or if they worsen, re-evaluation by a healthcare provider is necessary to rule out other causes of vaginitis.
