# Miconazole Vaginal Pessary
## Overview
Miconazole is an imidazole antifungal agent that inhibits the synthesis of ergosterol, a key component of fungal cell membranes. This leads to increased membrane permeability and cell death.
## Primary Indications
Treatment of vulvovaginal candidiasis (yeast infection).
## Adult Dosing
* **100 mg vaginal pessary:** Insert 1 pessary vaginally at bedtime for 7 consecutive days.
* **200 mg vaginal pessary:** Insert 1 pessary vaginally at bedtime for 3 consecutive days.
* **Higher strength formulations (e.g., 1200 mg single-dose):** Insert 1 single-dose pessary vaginally as a one-time treatment.
Specific dosing and duration depend on the product formulation available and local protocols.
## Pediatric Dosing
Safety and efficacy in pediatric patients have not been established for vaginal pessary formulations.
## Dose Adjustments
No specific dose adjustments are typically required for renal or hepatic impairment with vaginal administration, as systemic absorption is minimal.
## Contraindications
* Hypersensitivity to miconazole or any component of the formulation.
## Adverse Effects
* **Common:** Vaginal burning, itching, irritation, pelvic cramps, lower abdominal pain.
* **Less Common:** Headache, rash.
## Key Drug Interactions
* **CYP3A4 Inhibitors:** Miconazole is a potent inhibitor of CYP3A4. Concomitant use with drugs metabolized by CYP3A4 (e.g., certain statins, benzodiazepines, anticoagulants like warfarin) may increase the risk of toxicity from these drugs. Systemic absorption from vaginal administration is generally low, but caution is advised, especially with prolonged use or in patients with risk factors for systemic absorption.
* **Live Attenuated Vaccines:** Vaginal miconazole may reduce the efficacy of live attenuated vaccines (e.g., rotavirus vaccine). Separate administration by at least 2 days or consider delaying vaccination.
## Monitoring
* Monitor for signs and symptoms of local irritation or allergic reaction.
* Assess for symptom resolution (e.g., reduction in itching, burning, discharge).
* If symptoms persist after the course of treatment, consider alternative antifungal therapy and further evaluation for resistant strains or other diagnoses.
## Clinical Pearls
* Complete the full course of treatment as prescribed, even if symptoms improve sooner.
* Avoid sexual intercourse during treatment to prevent potential irritation and to ensure efficacy.
* Miconazole vaginal products can weaken latex condoms and diaphragms, reducing their effectiveness as contraceptive or STI prevention methods. Advise patients to use alternative birth control methods during treatment and for at least 72 hours after completion.
* Use the applicator provided to ensure proper insertion.
* Some formulations may cause staining of underwear or bed linens.
