# Miconazole Vaginal Pessary
## Overview
Miconazole is an imidazole antifungal agent that works by inhibiting the synthesis of ergosterol, a vital component of fungal cell membranes.
## Primary Indications
Vulvovaginal candidiasis (yeast infection).
## Adult Dosing
* **100 mg pessary:** Insert one pessary vaginally at bedtime for 7 consecutive nights.
* **200 mg pessary:** Insert one pessary vaginally at bedtime for 3 consecutive nights.
* **150 mg pessary:** Insert one pessary vaginally at bedtime as a single dose.
## Pediatric Dosing
Dosing in pediatric patients is not well established and should be based on clinical judgment and available product labeling. Consider adult formulations if appropriate for age and weight, and consult product-specific information.
## Dose Adjustments
No dose adjustments are typically required for renal or hepatic impairment.
## Contraindications
* Hypersensitivity to miconazole or any component of the formulation.
## Adverse Effects
Common adverse effects include vaginal itching, burning, irritation, and lower abdominal cramping.
## Key Drug Interactions
* **Warfarin:** Miconazole can inhibit CYP2C9, potentially increasing the effect and risk of bleeding with warfarin. Monitor INR closely.
* **CYP3A4 Substrates:** Potential for increased concentrations of drugs metabolized by CYP3A4.
## Monitoring
* Monitor for symptom relief (e.g., decrease in itching, burning, and discharge).
* Assess for and manage any adverse effects.
## Clinical Pearls
* Complete the full course of treatment, even if symptoms improve earlier.
* Avoid sexual intercourse during treatment to prevent potential reinfection or transmission to a partner.
* The pessary may weaken latex condoms and diaphragms. Consider alternative birth control methods during treatment.
