# Miconazole Vaginal Pessary
## Overview
Miconazole is an imidazole antifungal agent that disrupts fungal cell membrane synthesis. The vaginal pessary formulation is designed for localized treatment of vaginal yeast infections.
## Primary Indications
Treatment of vulvovaginal candidiasis (yeast infection).
## Adult Dosing
* **100 mg vaginal pessary:** Insert one pessary vaginally once daily at bedtime for 7 days.
* **200 mg vaginal pessary:** Insert one pessary vaginally once daily at bedtime for 3 days.
* **100 mg vaginal suppository:** Insert one suppository vaginally once daily at bedtime for 7 days.
* **Intravaginal cream (2%):** Insert one applicatorful (approximately 5g) vaginally once daily at bedtime for 7 days.
## Pediatric Dosing
Dosing in pediatric patients is not well-established. Consult with a pediatrician for appropriate management.
## Dose Adjustments
No specific dose adjustments are typically required for renal or hepatic impairment with vaginal administration due to minimal systemic absorption.
## Contraindications
* Hypersensitivity to miconazole or any component of the formulation.
## Adverse Effects
* **Common:** Vaginal burning, itching, irritation, lower abdominal cramping.
* **Less Common:** Headache, rash.
## Key Drug Interactions
* **CYP3A4 Inhibitors:** Miconazole is a potent inhibitor of CYP3A4. Concurrent use with CYP3A4 substrates may increase plasma concentrations of these drugs, potentially leading to serious adverse effects. Examples include statins, benzodiazepines, and certain anticoagulants. Advise patients to avoid or use caution with other medications.
* **Warfarin:** Miconazole can potentiate the anticoagulant effect of warfarin. Monitor INR closely.
## Monitoring
* Monitor for signs and symptoms of vaginal candidiasis.
* Assess for relief of itching, burning, and discharge.
* Monitor for any adverse effects.
## Clinical Pearls
* Complete the full course of treatment, even if symptoms improve earlier.
* Avoid sexual intercourse during treatment.
* Partners may require treatment if symptomatic.
* Application at bedtime can help minimize leakage.
* Use of tampons during treatment is not recommended.
* Pregnancy: While systemic absorption is minimal, consult with a healthcare provider regarding the safest treatment option during pregnancy.
