# Miconazole Vaginal Pessary
## Overview
Miconazole is a synthetic imidazole antifungal agent. It works by inhibiting the synthesis of ergosterol, a key component of fungal cell membranes, leading to increased cell membrane permeability and cell death.
## Primary Indications
Treatment of vulvovaginal candidiasis (yeast infection).
## Adult Dosing
* **100 mg vaginal pessary:** Insert one pessary vaginally at bedtime for 7 consecutive days.
* **200 mg vaginal pessary:** Insert one pessary vaginally at bedtime for 3 consecutive days.
* **For cream formulations (e.g., 2%):** 1 applicatorful (approximately 5 grams) inserted vaginally at bedtime for 7 days.
## Pediatric Dosing
* Dosing in pediatric patients is not well-established for vaginal pessaries. Safety and efficacy have not been determined. Consult with a pediatrician for appropriate management.
## Dose Adjustments
No dose adjustments are typically required for hepatic or renal impairment.
## Contraindications
* Hypersensitivity to miconazole or any component of the formulation.
## Adverse Effects
* **Common:** Vaginal burning, itching, irritation, lower abdominal cramping.
* **Less Common:** Headache, dizziness.
## Key Drug Interactions
* **CYP3A4 Substrates:** Miconazole is a potent inhibitor of CYP3A4. Concurrent use with sensitive CYP3A4 substrates (e.g., certain statins, benzodiazepines, calcium channel blockers, anticoagulants like warfarin) can lead to increased plasma concentrations of these drugs, potentially causing toxicity. Avoid concurrent use or use extreme caution, monitor closely, and consider dose reductions of the interacting drug.
## Monitoring
* Monitor for signs and symptoms of persistent or recurrent vaginal candidiasis.
* Monitor for adverse effects, particularly local irritation and signs of systemic drug interactions if concurrent CYP3A4 substrates are used.
## Clinical Pearls
* Complete the full course of treatment even if symptoms improve.
* Avoid sexual intercourse during treatment.
* If symptoms do not improve after completing treatment, or if they worsen, consult a healthcare provider.
* The pessary may weaken latex condoms and diaphragms, potentially leading to contraceptive failure and increased risk of sexually transmitted infections. Alternative contraception methods should be used during treatment and for at least 3 days after completion.
