# Miconazole Vaginal Pessary
## Overview
Miconazole is an imidazole antifungal agent. The vaginal pessary formulation is designed for localized treatment of vaginal yeast infections.
## Primary Indications
* Vulvovaginal candidiasis (yeast infection).
## Adult Dosing
* **100 mg vaginal pessary:** Insert 1 pessary vaginally at bedtime for 7 days.
* **200 mg vaginal pessary:** Insert 1 pessary vaginally at bedtime for 3 days.
* **Higher strength pessaries (e.g., 1200 mg single dose):** Insert 1 pessary vaginally as a single dose at bedtime.
Specific dosing and duration may vary based on product availability and patient factors. Follow the specific product's labeling.
## Pediatric Dosing
Safety and efficacy in pediatric patients (under 12 years) have not been established for vaginal pessary formulations. Use is generally not recommended in this age group unless specifically directed by a healthcare provider.
## Dose Adjustments
No dose adjustments are typically required for renal or hepatic impairment due to minimal systemic absorption.
## Contraindications
* Hypersensitivity to miconazole or any component of the formulation.
## Adverse Effects
* **Common:** Vaginal burning, itching, irritation, pelvic cramps.
* **Less Common:** Headache, rash.
## Key Drug Interactions
* **CYP3A4 Inhibitors:** Miconazole is a moderate inhibitor of CYP3A4. Concurrent use with drugs metabolized by CYP3A4 (e.g., certain statins, anticoagulants, immunosuppressants) may increase plasma concentrations and risk of toxicity. Careful monitoring or dose adjustment of the interacting drug may be necessary.
* **Latex Condoms/Diaphragms:** Miconazole vaginal products can weaken latex and rubber products. Advise patients to use alternative methods of contraception during treatment and for at least 72 hours after completion.
## Monitoring
* Monitor for signs and symptoms of vaginal candidiasis.
* Assess for and manage adverse effects.
* Monitor for potential drug interactions if the patient is on concomitant systemic medications metabolized by CYP3A4.
## Clinical Pearls
* Complete the full course of treatment as prescribed, even if symptoms improve sooner.
* Avoid sexual intercourse during treatment to prevent irritation and potential transmission.
* Miconazole vaginal products may cause local irritation.
* These products are generally safe for use during pregnancy, but consultation with a healthcare provider is recommended, especially in the first trimester.
