# Miconazole vaginal pessary
## Overview
Miconazole is a synthetic imidazole antifungal agent. It works by inhibiting the synthesis of ergosterol, a vital component of fungal cell membranes, leading to increased cell membrane permeability and fungal cell death.
## Primary Indications
Treatment of vulvovaginal candidiasis (yeast infection).
## Adult Dosing
* **100 mg pessary:** Insert one pessary vaginally at bedtime for 7 days.
* **200 mg pessary:** Insert one pessary vaginally at bedtime for 3 days.
* **1200 mg pessary:** Insert one pessary vaginally as a single dose.

## Pediatric Dosing
Safety and efficacy in pediatric patients have not been established for vaginal pessaries. Use in this population is generally not recommended without specific clinical indication and guidance.

## Contraindications
* Hypersensitivity to miconazole or any component of the formulation.

## Adverse Effects
Common adverse effects include vaginal burning, itching, irritation, and pelvic pain. Less common effects may include headache, rash, and urticaria.
## Key Drug Interactions
Miconazole is a moderate inhibitor of CYP3A4. Concomitant use with other medications metabolized by CYP3A4 may increase plasma concentrations of those drugs, potentially leading to toxicity. This includes, but is not limited to, warfarin, sulfonylureas, and certain immunosuppressants (e.g., cyclosporine, tacrolimus).
## Monitoring
* Monitor for signs and symptoms of vaginal candidiasis resolution.
* Assess for adverse effects, particularly local irritation.
* If symptoms persist or worsen after completing therapy, re-evaluate diagnosis and consider alternative treatment.
## Clinical Pearls
* Ensure the pessary is inserted as deeply as possible into the vagina, preferably at bedtime, to minimize leakage and maximize contact time.
* Avoid sexual intercourse during treatment.
* Partners may require treatment if symptomatic.
* The inactive ingredients in the pessary may interact with latex condoms or diaphragms, reducing their effectiveness. Advise patients to use alternative methods of contraception during treatment.
