# Miconazole vaginal pessary
## Overview
Miconazole is an imidazole antifungal agent used to treat vaginal yeast infections.
## Primary Indications
Treatment of vulvovaginal candidiasis (yeast infection).
## Adult Dosing
Available as 100 mg, 200 mg, and for combination therapy, a 1200 mg pessary.
* **100 mg pessary:** Insert one pessary vaginally at bedtime for 7 consecutive nights.
* **200 mg pessary:** Insert one pessary vaginally at bedtime for 3 consecutive nights.
* **1200 mg pessary:** Insert one pessary vaginally at bedtime as a single dose.
The optimal duration of therapy may vary based on individual response and severity of infection.
## Pediatric Dosing
Data for pediatric use of miconazole vaginal pessaries is limited. Use in pediatric patients should be based on clinical judgment and available product information, with caution. Consult specific product labeling for any available pediatric recommendations.
## Dose Adjustments
No dose adjustments are typically required for renal or hepatic impairment for vaginal administration.
## Contraindications
Hypersensitivity to miconazole or any component of the formulation.
## Adverse Effects
Common adverse effects include vaginal burning, itching, irritation, and abdominal cramping. Allergic reactions can occur.
## Key Drug Interactions
* **Warfarin:** Miconazole can inhibit CYP2C9, leading to increased warfarin levels and risk of bleeding. Closely monitor INR and consider alternative antifungal agents if patient is on warfarin.
* **CYP3A4 Substrates:** Miconazole can inhibit CYP3A4, potentially increasing plasma concentrations of drugs metabolized by this enzyme (e.g., certain calcium channel blockers, immunosuppressants). Vaginal absorption is generally low, but caution is advised.
## Monitoring
* Monitor for signs and symptoms of infection resolution.
* Monitor for adverse effects, particularly signs of hypersensitivity or vaginal irritation.
* If patient is on warfarin, monitor INR closely.
## Clinical Pearls
* Complete the full course of treatment, even if symptoms improve earlier.
* Avoid sexual intercourse during treatment to prevent irritation and potential transmission.
* Consider using sanitary pads to manage potential leakage of the pessary or medication.
* For recurrent or persistent infections, further evaluation may be necessary.
