# Miconazole vaginal pessary
## Overview
Miconazole is an imidazole antifungal agent. Vaginal pessaries are inserted into the vagina for local treatment.
## Primary Indications
Vulvovaginal candidiasis (yeast infection).
## Adult Dosing
* **100 mg vaginal pessary:** Insert 1 pessary vaginally once daily at bedtime for 7 days.
* **200 mg vaginal pessary:** Insert 1 pessary vaginally once daily at bedtime for 3 days.
* **100 mg vaginal suppository:** Insert 1 suppository vaginally once daily at bedtime for 7 days.
* **200 mg vaginal suppository:** Insert 1 suppository vaginally once daily at bedtime for 3 days.
* **A combination product containing miconazole nitrate 2% and hydrocortisone 0.5% vaginal cream:** Follow specific product labeling; typically applied once or twice daily for a specified duration.
## Pediatric Dosing
Safety and efficacy in pediatric patients have not been established. Use with caution and consult with a pediatrician.
## Dose Adjustments
No dose adjustments are typically needed based on renal or hepatic impairment as systemic absorption is minimal.
## Contraindications
* Known hypersensitivity to miconazole or any component of the formulation.
* Use with latex condoms or diaphragms: Miconazole can weaken latex and rubber products, increasing the risk of breakage.
## Adverse Effects
* **Common:** Vaginal burning, itching, irritation, pelvic cramps.
* **Less Common:** Headache, rash.
## Key Drug Interactions
* **Warfarin:** Miconazole (especially oral formulations, but topical may have some interaction) can inhibit CYP2C9, increasing warfarin levels and the risk of bleeding. Monitor INR closely if co-administered.
* **CYP3A4 Substrates:** Miconazole can inhibit CYP3A4, potentially increasing the plasma concentrations of drugs metabolized by this enzyme (e.g., certain statins, benzodiazepines, calcium channel blockers). Systemic absorption is generally low with vaginal products, but caution is advised.
## Monitoring
* Clinical signs and symptoms of vulvovaginal candidiasis.
* Monitor for signs of local irritation or allergic reaction.
## Clinical Pearls
* Treatment duration varies by product strength and patient response.
* Complete the full course of therapy even if symptoms improve.
* Avoid sexual intercourse during treatment.
* Miconazole can potentially weaken latex contraceptives.
* Patient education regarding proper insertion technique is important.
