# Miconazole Vaginal Pessary
## Overview
Miconazole is an imidazole antifungal agent. It works by inhibiting the synthesis of ergosterol, a vital component of fungal cell membranes.
## Primary Indications
Treatment of vulvovaginal candidiasis (yeast infection).
## Adult Dosing
* **100 mg vaginal pessary:** Insert one pessary vaginally once daily at bedtime for 7 days.
* **200 mg vaginal pessary:** Insert one pessary vaginally once daily at bedtime for 3 days.
* **Vaginal suppository (100mg):** Insert one suppository vaginally at bedtime for 7 days.
* **Vaginal suppository (200mg):** Insert one suppository vaginally at bedtime for 3 days.
* **Vaginal suppository (single dose 1200mg):** Insert one suppository vaginally at bedtime.
## Pediatric Dosing
Safety and efficacy in pediatric patients have not been established for the vaginal pessary formulation. Use should be based on clinical judgment and physician recommendation.
## Dose Adjustments
No specific dose adjustments are typically required for renal or hepatic impairment, as systemic absorption is minimal.
## Contraindications
* Hypersensitivity to miconazole or any component of the formulation.
* Concurrent use with warfarin (increased risk of bleeding).
## Adverse Effects
Common: Burning sensation, itching, irritation of the vagina and vulva.
Less common: Abdominal pain, headache, rash.
## Key Drug Interactions
* **Warfarin:** Miconazole can inhibit the metabolism of warfarin, increasing the anticoagulant effect and risk of bleeding. Concurrent use is contraindicated.
* **CYP3A4 substrates:** Miconazole is a potent inhibitor of CYP3A4. Avoid concomitant use with drugs metabolized by this enzyme (e.g., certain calcium channel blockers, benzodiazepines, statins) due to potential for increased systemic concentrations and toxicity.
## Monitoring
* Monitor for signs and symptoms of vaginal candidiasis, including discharge, itching, and erythema.
* Monitor for adverse effects.
* If using concurrently with other systemic antifungals, monitor for potential drug interactions.
## Clinical Pearls
* Complete the full course of treatment even if symptoms improve.
* Avoid intercourse during treatment as the product may weaken latex condoms and diaphragms.
* If symptoms do not improve after completing treatment, or if symptoms worsen, consult a healthcare provider.
* The applicator can be used to insert the pessary. Lubricate the applicator with petroleum jelly if needed.
