# Miconazole Vaginal Pessary
## Overview
Miconazole is an imidazole antifungal agent that works by inhibiting the synthesis of ergosterol, a crucial component of fungal cell membranes. This leads to increased cell membrane permeability and leakage of intracellular contents, ultimately causing fungal cell death.
## Primary Indications
Treatment of vulvovaginal candidiasis (yeast infection).
## Adult Dosing
* **100 mg vaginal pessary:** Insert 1 pessary vaginally at bedtime for 7 days.
* **200 mg vaginal pessary:** Insert 1 pessary vaginally at bedtime for 3 days.
* **100 mg vaginal suppository:** Insert 1 suppository vaginally at bedtime for 7 days.
* **200 mg vaginal suppository:** Insert 1 suppository vaginally at bedtime for 3 days.
* **Loading dose option (less common):** 1200 mg vaginal suppository (single dose).
## Pediatric Dosing
Safety and efficacy in pediatric patients have not been established for miconazole vaginal pessaries.
## Dose Adjustments
No dose adjustments are typically required based on renal or hepatic impairment.
## Contraindications
* Hypersensitivity to miconazole or any component of the formulation.
* Use with CYP3A4 inhibitors (e.g., cisapride, pimozide, quinidine, oral midazolam, triazolam) due to the risk of serious adverse events.
## Adverse Effects
* **Common:** Vaginal burning, itching, irritation, pelvic pain, headache.
* **Less Common:** Rash, urticaria, Stevens-Johnson syndrome (rare).
## Key Drug Interactions
Miconazole is a potent inhibitor of CYP3A4 and CYP2C9. Concurrent use with CYP3A4 substrates such as cisapride, pimozide, quinidine, oral midazolam, and triazolam is contraindicated. Caution and potential dose reduction are advised for other CYP3A4 substrates (e.g., statins, calcium channel blockers, cyclosporine, tacrolimus, sirolimus, certain anticoagulants like warfarin).
## Monitoring
* Monitor for signs and symptoms of vulvovaginal candidiasis.
* Assess for and manage adverse effects, particularly local irritation.
* Monitor for signs of drug interactions if used concurrently with other medications.
## Clinical Pearls
* Complete the full course of therapy as directed, even if symptoms improve sooner.
* Avoid sexual intercourse during treatment to prevent irritation and potential transmission to a partner.
* Tampons should not be used during treatment.
* The accompanying applicator can be used for insertion. Clean the applicator after each use.
* If symptoms persist after completing therapy, re-evaluate the diagnosis and consider alternative treatment.
* If recurrent infections occur, further investigation for underlying causes may be warranted.
