# Miconazole vaginal pessary
## Overview
Miconazole is an imidazole antifungal agent that inhibits fungal ergosterol synthesis, disrupting fungal cell membrane integrity.
## Primary Indications
Treatment of vulvovaginal candidiasis (yeast infection).
## Adult Dosing
* **100 mg vaginal pessary:** Insert one pessary vaginally at bedtime for 7 consecutive days.
* **200 mg vaginal pessary:** Insert one pessary vaginally at bedtime for 3 consecutive days.
* **High potency (e.g., 1200 mg) vaginal suppository:** Insert one suppository vaginally at bedtime as a single dose.
The specific regimen may vary based on the product formulation and clinical guidelines.
## Pediatric Dosing
* **Children 12 years and older:** Use adult dosing.
* **Children younger than 12 years:** Safety and efficacy have not been established. Use cautiously if deemed necessary by a healthcare provider.
## Dose Adjustments
No dose adjustments are typically required for renal or hepatic impairment for vaginal administration.
## Contraindications

## Key Drug Interactions
* **CYP3A4 inhibitors:** Miconazole is a potent inhibitor of CYP3A4. Concurrent use with oral anticoagulants (e.g., warfarin) may increase the risk of bleeding. ## Monitoring

## Clinical Pearls

* Avoid sexual intercourse during treatment.
