# Miconazole Vaginal Pessary
## Overview
Miconazole is a synthetic imidazole antifungal agent. It works by inhibiting the synthesis of ergosterol, a vital component of fungal cell membranes.
## Primary Indications
Treatment of vulvovaginal candidiasis (yeast infection).
## Adult Dosing
* **100 mg vaginal pessary:** Insert one pessary vaginally at bedtime for 7 consecutive nights.
* **200 mg vaginal pessary:** Insert one pessary vaginally at bedtime for 3 consecutive nights.
* **400 mg vaginal pessary:** Insert one pessary vaginally at bedtime for 1 night.
## Pediatric Dosing
Safety and efficacy in pediatric patients have not been established. Use is generally not recommended.
## Dose Adjustments
No specific dose adjustments are required for renal or hepatic impairment as systemic absorption is minimal.
## Contraindications
* Hypersensitivity to miconazole or any component of the formulation.
* Concurrent use with certain medications (see Key Drug Interactions).
## Adverse Effects
* Vaginal burning or irritation
* Pruritus
* Rash
* Pelvic cramps
* Headache
## Key Drug Interactions
* **CYP3A4 Inhibitors:** Miconazole is a potent inhibitor of CYP3A4. Concurrent use with miconazole oral gel or other formulations that lead to significant systemic absorption can increase plasma concentrations of drugs metabolized by CYP3A4, such as:
* Certain statins (e.g., simvastatin, atorvastatin)
* Certain benzodiazepines (e.g., midazolam, triazolam)
* Certain opioids (e.g., fentanyl)
* Certain anticoagulants (e.g., warfarin)
* Certain immunosuppressants (e.g., cyclosporine, tacrolimus)
* Erectile dysfunction agents (e.g., sildenafil)
* Calcium channel blockers (e.g., amlodipine)
* Certain antiarrhythmics (e.g., amiodarone)
* Certain antiretrovirals (e.g., ritonavir)
* Certain antineoplastics.

* **Dutasteride:** Miconazole can inhibit CYP3A4, leading to increased dutasteride levels.
## Monitoring
* Monitor for signs and symptoms of therapeutic response.
* Monitor for adverse effects, particularly local irritation.
* Monitor for potential drug interactions if systemic absorption is suspected or if concurrent medications are used.
## Clinical Pearls
* Ensure the pessary is inserted as deeply as possible into the vagina for optimal effect.
* Avoid sexual intercourse during treatment.
* Treatment may be continued during menstruation.
* If symptoms persist after completion of therapy, re-evaluate the diagnosis and consider alternative treatments.
