# Miconazole Vaginal Pessary
## Overview
Miconazole is a synthetic imidazole antifungal agent. It is available as a vaginal pessary for the treatment of vulvovaginal candidiasis.
## Primary Indications
Treatment of vulvovaginal candidiasis (yeast infection).
## Adult Dosing
* **100 mg vaginal pessary:** Insert 1 pessary vaginally at bedtime for 7 consecutive nights.
* **200 mg vaginal pessary:** Insert 1 pessary vaginally at bedtime for 3 consecutive nights.
* **100 mg vaginal suppository (may be used interchangeably with pessary):** Insert 1 suppository vaginally at bedtime for 7 consecutive nights.
## Pediatric Dosing
Use in pediatric patients is generally not recommended due to limited safety and efficacy data. Consult with a pediatrician or infectious disease specialist for guidance.
## Dose Adjustments
No dose adjustments are typically required for renal or hepatic impairment with vaginal administration.
## Contraindications
* Known hypersensitivity to miconazole or any component of the formulation.
## Adverse Effects
* **Common:** Vaginal burning, itching, irritation, redness, pelvic cramps.
* **Less Common:** Headache, rash.
## Key Drug Interactions
* **CYP3A4 Inhibitors:** Miconazole is a potent inhibitor of CYP3A4. Concurrent use with oral midazolam or triazolam can lead to exaggerated and prolonged sedative effects. Avoid coadministration. If coadministration is unavoidable, monitor closely for CNS depression.
* **Warfarin:** Miconazole may increase the anticoagulant effect of warfarin. Monitor INR closely, especially if higher doses or prolonged treatment are used. Dose adjustment of warfarin may be necessary.
## Monitoring
* Monitor for signs and symptoms of vulvovaginal candidiasis to assess treatment efficacy.
* Monitor for signs of local irritation or allergic reaction.
## Clinical Pearls
* Complete the full course of therapy even if symptoms improve earlier.
* Avoid sexual intercourse during treatment to prevent potential reinfection and to avoid irritation from the pessary.
* Pessaries may weaken latex condoms and diaphragms, rendering them less effective. Advise patients to use alternative methods of contraception during treatment and for at least 72 hours after completing therapy.
* If symptoms persist after completing treatment, or if they worsen, re-evaluate for alternative diagnoses or resistant organisms.
